The subject of the contract is the service specified in the Common Procurement Vocabulary (CPV) with the code and name: Main Dictionary "90611000-3" "Street cleaning services", "90612000-0" "Street sweeping services", "90620000-9" "Snow removal services "And" 90630000-2 "" De-icing services ", consisting of ongoing cleaning, snow removal and de-icing of the roadway, roadsides, bays, parking lots, squares and pedestrian routes along the entire width of the city of Łowicz along roadways, turnouts, intersections, islets, etc. and on the entire surface as well as sidewalks, pavement paths, pedestrian and traffic routes, squares, parking lots, parking bays, bays and shoulders along the roads of the city of Łowicz, parks, squares and other designated areas along the entire width and on the entire surface in a way that ensures that they are kept permanently clean during the period from 1.1.2019 to 31.12.2021. 2. Ordering mode - open tender

1. The subject of the contract is the service specified in the Common Procurement Vocabulary (CPV) with the code and name: Main Dictionary "90611000-3" "Street cleaning services", "90612000-0" "Street sweeping services", "90620000-9" "Snow removal services "And" 90630000-2 "" De-icing services ", consisting of ongoing cleaning, snow removal and de-icing of the roadway, roadsides, bays, parking lots, squares and pedestrian routes along the entire width of the city of Łowicz along roadways, turnouts, intersections, islets, etc. and on the entire surface as well as sidewalks, pavement paths, pedestrian and traffic routes, squares, parking lots, parking bays, bays and shoulders along the roads of the city of Łowicz, parks, squares and other designated areas along the entire width and on the entire surface in such a way as to ensure that they are kept permanently clean during the period from 1.1.2019 to 31.12.2021.

2. The Detailed Description of the Object of the Contract is specified in Annex No. 1 to the ToR.

Conditions for participating in the contract award procedure may be applied by Contractors who:

1. are not subject to exclusion in the circumstances referred to in art. 24 paragraph 1 points 12-23 and para. 5 points 1 and 8 of the Public Procurement Law;

List of statements and documents confirming the absence of grounds for exclusion.

In order to confirm the lack of grounds for excluding the Contractor from participation in the procedure, one should submit:

a) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submitting bids, or another document confirming that the Contractor has entered into an agreement with the relevant tax authority regarding the repayment of these debts together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full of the decision of the competent authority,

b) a certificate of the competent organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting offers, or another confirmation document, that the Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables, together with any interest or fines, in particular, he has obtained a legal exemption, deferment or installment of overdue payments or withholding in full the execution of the decision of the competent authority.

If the Contractor relies on the ability or situation of other entities to confirm the absence of grounds for excluding the Contractor from participation in the procedure, the Employer will request the Contractor to submit the documents listed in point ab also for these entities.

c) if the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in point ab - submits a document or documents issued in the country in which the Contractor has its registered office or place of residence - in accordance with Dz. VI para. 3.2. point cid SIWZ

Requirements for the bid bond are specified in section VIII of the ToR

Place and deadline for submitting bids

1) bids must be submitted to the secretary's office of the Municipal Office in Łowicz, Pl. Stary Rynek 1, building "A", room 7 until November 26, 2018 to 10:00

The contract does not include a framework agreement.

The order does not include a dynamic purchasing system

The Awarding Entity does not envisage selecting the most advantageous bid by means of an electronic auction.

The awarding entity does not envisage the award of contracts referred to in art. 67 section 1 item 6 and 7 of the Public Procurement Law.
