Rump. 1 – Patient body wash wipes

The Purchaser does not allow partial submissions.

3.3. Requirements of the Ordering Party regarding the subject of the contract: (if applicable)

1) the medical device offered must have attestations, certificates and be registered in accordance with legal regulations requiring possession of attestations, certificates and registration,

2) the medical device offered must be placed on the market and used in accordance with

with the requirements of the Act of 7 April 2022 on medical devices (Journal of Laws of 2022, item 974),

3) the medical device offered must comply with the standards or technical specifications applicable to that device;

In the event that the offered medical device does not require notification to the President of the Office for Registration of Medicinal Products, Medical Devices and Biocidal Products, the Contractor is obliged to carry out at his own expense and on his own account all activities necessary to meet all legal requirements related to placing the product on the market and use in the Republic of Poland. The Contractor is obliged to perform such activities within the time limits resulting from applicable regulations and provide the Employer with relevant documents confirming the performance of such activities within those time limits.

3.4. In cases where trademarks, patents or origin, source or specific process that characterizes certain products or services provided by a particular contractor are indicated in the description of the subject of the contract, this means that the Contracting Authority cannot describe the subject of the contract in sufficiently precise and understandable, however, in any such situation the Contracting Authority allows a solution equivalent to those described in the above. In such situations, possible indications for trademarks, patents, origin, source or specific process that characterizes certain products or services provided by a particular contractor shall be read together with the words “or equivalent”.

When describing the subject of the contract by reference to standards, technical assessments, technical specifications and technical reference systems referred to in Article 101 (1) (2) and (3), the Contracting Authority shall allow solutions equivalent to those described In such situations, the possible indications should be read together with the words “or equivalent”.

The Contracting Authority shall not provide for the award of contracts referred to in Article 214 (1) (7) of the Law Pzp.

13. Description of the criteria to be guided by the Contracting Authority when selecting the offer, including the weighting of those criteria and how to assess the tenders

The evaluation of tenders will be carried out on the basis of the criteria set out in the SWZ and their weights.

Offers will be scored.

During the evaluation of the offers, the subsequent considered and evaluated offers are awarded points for the following criteria according to the following rules:

CRITERIUM PRICE Weight: 60%

CRITERIUM THERMIN OF THE COMPLETE REQUIREMENT Weight: 40%
