1. The subject of the contract is the Computerization of SP ZOZ in Lesko as part of the project entitled "Development of e-services (PSIM) in the area of impact of the Independent Public Health Care Team in Lesko".
2. The subject of the contract includes 5 indivisible packages, for which the Ordering Party allows the possibility of submitting partial offers, with the proviso that the offer in each of the packages should be full and should meet the detailed requirements specified in the price forms constituting Annex 2 to the SWZ, the form of required technical, functional and utility parameters constituting Annex 3.1 - 3.5 to the SWZ as well as the requirements contained in Chapter III of this specification.
