1. The subject of the order is the gradual delivery of orthopedic implants and prostheses. The subject of the order was divided into 18 lots (Packages No. 1-18).

2. A detailed description of the subject of the contract in individual packages is set out in Annex 1A to the ToR - an assortment and price form.

3. The implants offered must meet the legal requirements for their use in Poland, in particular the Act on Medical Devices of May 20, 2010. Dz. Of Laws of 2017, item 211, as amended d.

4. The unit packaging of implant components and invoices must show the trade name, catalog number as well as the series and expiry date.

5. The contractor is obliged to:

1) creating an implant warehouse on the premises of the Employer;

2) providing complete sets of instruments for installing implants and battery drives, enabling the performance of treatments according to the offered technology

Hip joint endoprosthesis enabling adjustment of the offset by lateralization and the use of two-mobile anti-luxation acetabulars. A detailed description and quantities are specified in the Assortment and Price Form (Annex 1A to the Terms of Reference).

Set for primary and revision hip replacement (acetabular and femoral part). A detailed description and quantities are specified in the Assortment and Price Form (Annex 1A to the Terms of Reference).
