4.1. The subject of the contract is the successive DELIVERY OF SINGLE-USE AND REUSABLE MEDICAL DEVICES (hereinafter referred to as medical devices)
4.2. The type, minimum parameters and quantities are specified in the Detailed Offer and Price Form which is attached as Appendix No. 1 to the Terms of Reference.
4.3. The validity period of the offered products cannot be shorter than 12 months, counted from the date of delivery.
4.4. The Contractor is obliged to deliver medical devices with information (leaflet) in accordance with the applicable law.
4.5. The subject matter of the contract must be marked by the manufacturers in such a way that it is possible to identify both the product and the manufacturer.
4.6. The subject of the contract classified as a medical device must meet the requirements of the Act of 7 April 2022 on medical devices. It must be authorised for marketing
and use in the Polish in accordance with applicable regulations.
