The subject of the contract are services consisting in periodic technical inspections, maintenance and ongoing repairs of equipment used by the Ordering Party, i.e. 1 pc. magnetic resonance imaging and 2 pcs. X-ray machines according to the following parts:

Task 1: Magnetic resonance inspection and maintenance services

Task 2: Inspection and maintenance services for X-ray machines

A detailed description of the subject of the contract has been listed and described in the "Price form" (Appendix No. 2), "Scopes of activities" (Attachment No. 2a) and in the contract template (Attachment No. 3) constituting an integral part of the Terms of Contract.

The contractual conditions for the performance of the public contract in question are included in the model contract, which is an integral part of the SWZ. The Contractor undertakes to sign a contract for entrusting the processing of personal data (in accordance with Annex 10 to the SWZ).
