The subject of the contract is construction work involving the construction of an intersection at the intersection of Roosevelt and Piłsudskiego Streets in Zabrze, including: construction of a roundabout, reconstruction of existing streets; reconstruction and construction of pavements, bicycle paths, parking spaces, entrances, turning point; reconstruction and construction of street lighting; reconstruction and construction of drainage, reconstruction and protection of colliding utility networks (gas, teletechnical, electric, water and sewage). This contract is the second stage of reconstruction of Piłsudskiego Street in Zabrze. The order is included in the scope of the task "Expansion of the intersection of ul. Roosevelt from ul. Piłsudskiego - construction of a roundabout, reconstruction of Piłsudskiego Street from the intersection with ul. Roosevelt to the railway viaduct, construction of lighting for pedestrian crossings along ul. 3rd Maja at the height of ul. Luther, Padlewski, Florian, Warsaw Heroes, Sądowa, Franklin Roosevelt ", financed under the Local Government Road Fund.

This contract is the second stage of reconstruction of Piłsudskiego Street in Zabrze.

The detailed description of the subject of the contract consists of:

1) Design documentation (DP):

a. Decision No. 1/2015 of 2.11.2015 authorizing the implementation of a road investment;

b. Construction project;

c. Executive design.

2) Technical Specifications of Performance and Acceptance of Construction Works (STWiORB);

3) Bill of Quantity (PR).

The contracting authority envisages the award of contracts referred to in art. 67 section 1 item 6 of the Public Procurement Law - consisting in the repetition of similar construction works whose need arose at the stage of the investment or operation, up to an amount of not more than 10% of the value of the basic contract.

In order to pre-confirm the fulfillment by the contractor of the conditions for participation in the procedure and the lack of grounds for excluding the contractor from participation in the procedure, the Employer requests:

- the uniform European procurement document, valid as at the date of submission of tenders, drawn up in accordance with the template of the standard form set out in the implementing regulation of the European Commission issued pursuant to art. 59 clause 2 of Directive 2014/24 / EU, hereinafter referred to as the "Single Document". The template of the Single Document submitted by the Employer is in accordance with Commission Implementing Regulation (EU) 2016/7 of 5.1.2016 establishing a standard form for the single European procurement document. The template of the Single Document submitted by the Employer is in accordance with Commission Implementing Regulation (EU) 2016/7 of 5.1.2016 establishing a standard form for the single European procurement document. The Contractor is obliged to complete the Uniform Document to the extent that the Employer is able to verify that the Contractor meets the conditions for participation in the procedure and is not subject to exclusion to the extent described in this ToR. The contractor fills in EAT, creating an electronic document, using the available tools. The contractor may use the ESPD tool, the electronic JED / ESPD filling tool available on the Public Procurement Office website (link to the tool: http://espd.uzp.gov.pl) or other available tools or software that allow you to fill in the EAT and create electronic document. The Purchaser allows in particular the following formats of sent data: .pdf, .doc, .docx, .odt. After completing the EAT, please save and sign with a qualified electronic signature, then attach to the offer in accordance with the ToR requirements. The Contractor, which refers to the resources of other entities, in order to demonstrate the absence of grounds for exclusion and compliance with them, to the extent that it refers to their resources, the conditions for participation in the proceedings also submits Uniform Documents concerning these entities. If Contractors jointly apply for a contract, the Single Document shall be submitted by each of the Contractors jointly applying for the contract. This document confirms the fulfillment of the conditions for participation in the procedure and the lack of grounds for exclusion.
