NHS Chorley and South Ribble Clinical Commissioning Group and NHS Greater Preston Clinical Commissioning Group are commissioning a number of nursing care beds for patients that require 24 hour supervision during their recuperation, this provision will be across a number of sites (3 separate sites are planned initially, with 5 beds in each site) enabling people to be as close to their community as possible, offering patient centred care that prevents admission, is clinically effective, outcome focussed and supports independence. The aim is to facilitate the transition to functional independence where possible so that each patient can return to their usual place of residence within a pre-defined period of time as detailed in their care and support plan.
This service will provide a subacute bed-based intermediate care within the Greater Preston area, for patients requiring nursing care and a range of short term treatment, complex discharge planning and rehabilitative care. This is both step up provision to prevent unnecessary acute hospital admissions and premature admissions to long term care and step down provision for patients from acute hospital settings for recuperation and rehabilitation to support timely discharge from hospital.
The commissioner wishes to contract for a quantity of 5 block beds at 1 site with an option to flex this number as required from time to time.
