Description:
Modernization of imaging diagnostics in SPZOZ in Gostyń

The subject of the contract is: Modernization of imaging diagnostics in SPZOZ in Gostyń. The order was divided into 4 packages: Package I - purchase of a computer tomograph,

Package II – purchase of a musculoskeletal and pulmonary X-ray machine, Package III – purchase of an X-ray machine with arm C, Package IV – purchase of a mammograph.

The place of performance is SPZOZ in Gostyń, Pl. K. Marcinkowskiego 8/9, 63-800 Gostyń.

Purchase of a CT scanner

The scope of the contract includes:

1) purchase, delivery, unloading, bringing, assembly of devices and commissioning in the place indicated by the Ordering Party together with all components and affiliations of the computed tomograph, X-ray apparatus – C-arm, bone and pulmonary X-ray machine (constituting the equipment of the X-ray laboratory) and mammograph with their equipment, accessories, auxiliary and cooperating devices and software and adaptation of rooms to the applicable requirements in this respect,

2) training in the use of the system for doctors, technicians and medical physicists at the headquarters of SPZOZ in Gostyń, Pl. K. Marcinkowskiego 8/9, Gostyń within the time agreed with the Ordering Party after launching all elements of the subject of the contract,

3) provision of warranty / maintenance services for a period of 24 months, counted from the date of signing the final delivery and acceptance protocol by both parties,

6. The Contractor is obliged to provide the necessary documentation related to the equipment and the adaptation carried out in order to submit it by the Contracting Authority to the Provincial Sanitary and Epidemiological Station. The Contracting Authority reserves the right to request the Contractor's assistance at every stage of obtaining a permit to use the studio,

7. Connection of the devices constituting the subject of the order to the system (software) owned by SPZOZ in Gostyń, Pl. K. Marcinkowskiego 8/9, 63-800 Gostyń. The ordering party requires the integration of the supplied equipment with its system (software) in the field of downloading and sending patient data. The contractor must include the necessary licences, installation and configuration services and maintenance services,

8. The devices being the subject of the supply are to be delivered with the installed software and licenses necessary for the operation of the device and its equipment for an indefinite period. The Contractor guarantees that the licenses provided to the Ordering Party are free from legal defects and are not encumbered with the rights of third parties and are non-exclusive licenses,

9. The Ordering Party shall accept the subject of the order by signing the final delivery and acceptance protocol, after proper delivery, assembly, installation and commissioning of the subject of the contract.

10. The Contractor, when assembling, installing and commissioning the subject of the contract in a room designated by the Contracting Entity, shall be obliged to leave these rooms in a condition not deteriorated in relation to the condition before the performance of the action. Installation, assembly and commissioning of the subject of the contract must be carried out with special care based on the currently applicable provisions of the law of the Republic of Poland and European Union Directives.

11. The facilities and necessary equipment offered must have the requisite approval for use in health care facilities and

about the required approval for use and marketing in the Polish i.e. CE certificate / declaration of conformity in accordance with the Regulation of the Minister of Health on essential requirements and procedures for conformity assessment of medical devices. Medical devices being the subject of the order should be understood as medical devices within the meaning of the Act of 7 April 2022 on medical devices (consolidated text, Journal of Laws of 2022, item 974).

Purchase of a musculoskeletal X-ray machine

The scope of the contract includes:

1) purchase, delivery, unloading, bringing, assembly of devices and commissioning in the place indicated by the Ordering Party together with all components and affiliations of the computed tomograph, X-ray apparatus – C-arm, bone and pulmonary X-ray machine (constituting the equipment of the X-ray laboratory) and mammograph with their equipment, accessories, auxiliary and cooperating devices and software and adaptation of rooms to the applicable requirements in this respect,

2) training in the use of the system for doctors, technicians and medical physicists at the headquarters of SPZOZ in Gostyń, Pl. K. Marcinkowskiego 8/9, Gostyń within the time agreed with the Ordering Party after launching all elements of the subject of the contract,

3) provision of warranty / maintenance services for a period of 24 months, counted from the date of signing the final delivery and acceptance protocol by both parties,
