The subject of the contract is the provision of comprehensive cleaning services in the facilities of the Pulmonology and Thoracic Surgery Center in Bystra. The order consists of 2 parts:

PART 1 – provision of comprehensive cleaning services in the facilities of the Pulmonology and Thoracic Surgery Center in Bystra, i.e.: cleaning hospital facilities, administration, intra-hospital transport, i.e. items related to the subject of the contract (including waste, underwear, etc.), service of ward cookers, cloakroom service for patients.

PART 2 – provision of comprehensive cleaning services in the Apartamenty Bystra Facility (so-called hotel cleaning) along with cleaning of small external architecture – by swimming pools, brine graduation tower, inhalatorium (benches, tables, deckchairs) and public toilet.

A detailed description of the subject of the contract (OPZ) can be found in Annex 2 to the SWZ (Part 1 and Part 2) and must be carried out in accordance with the conditions set out in the draft contract templates. No. 10 and No. 11.

The subject of the contract within the scope of PART 1 is the provision of comprehensive cleaning services in the facilities of the Pulmonology and Thoracic Surgery Center in Bystra, i.e.: cleaning of hospital facilities, administration, intra-hospital transport, i.e. items related to the subject of the contract (including waste, underwear, etc.), operation of ward cookers, cloakroom service for patients. A detailed description of the subject of the contract (OPZ) can be found in Annex 2 to the SWZ concerning Part 1 and must be performed in accordance with the conditions set out in the draft contract template Annex 10 to the SWZ concerning Part 1. The contracting authority informs that it provides for the obligation to conduct an on-site inspection by entities reporting their willingness to participate in the proceedings. The date and rules of the vision are indicated in Chapter I, paragraph 5 of the SWZ. J. A site visit will be drawn up. A protocol signed by both parties, which will constitute a confirmation of the completion of the on-site visit. No. 8 to SWZ. In case of failure to inspect the premises. offer Execution. will be rejected pursuant to Article 226(1)(18) of the Public Procurement Law. demands that the Contractors pay a bid bond in the amount of PLN 137,374.35 - the requirements for the bid bond are described in Chapter II, paragraph 7 of the SWZ. Order. pursuant to Article 95(2) of the Public Procurement Law specifies that the requirement to employ persons by the Contractor or its subcontractor on the basis of an employment relationship applies to the performance by these persons of activities related to the performance of this contract, i.e. maintaining cleanliness in CPiT facilities in Bystra - the obligation is described in Chapter I(15) of the SWZ and in Annex 10 to the SWZ - contract Part 1. Before signing the contract Zamaw. requires the lodging of a performance bond in the amount of 5% of the total price specified in the offer. Rules for lodging security properly. Execution. the agreements are indicated in Chapter III, paragraph 7 of the SWZ.

Order. before selecting the most advantageous offer, call the Contractor whose offer was rated the highest to submit within the prescribed period, not shorter than 10 days from the date of the call, current as at the date of submission of subjective evidence indicated in Chapter II, paragraph 6 SWZ - II.
