1. The subject of the contract is: Purchase and delivery of endoprostheses, orthopedic implants, materials fusing bone fractures along with the provision of instruments for the Dr. Ludwik Rydygier Provincial Hospital in Suwałki

2. For a detailed description of the item of the order, see Annex 5 to the SWZ.

3. The subject of the contract shall include the provision of the instrumentarium according to the rules set out in Annex 2a to the SWZ.

4. Delivery of the subject of the order will take place on the basis of depositing a minimum of 1 piece of each type of assortment specified in the Assortment and Price Form constituting Annex 5 to the SWZ. Detailed rules for the functioning of the deposit are set out in Annex 2b to the SWZ.

5. The Contractor shall, in the framework of the implementation of this Agreement, provide free training of the Ordering Party's medical staff in terms of operational techniques in the scope of the contract at the premises of the Contracting Authority

Hip joint endoprostheses in the amount and assortment specified in Annex 5 to the SWZ

Temporary implants and cements in the amount and assortment specified in Annex 5 to the SWZ

Knee and shoulder joint fixing implants in the number and assortment specified in Annex 5 to the SWZ

Vertebroplasty kit for vertebroplasty of the thoracic and lumbar vertebrae of the spine with a biopsy needle in the amount and assortment specified in Annex 5 to the SWZ

Spine implants - a system for stabilizing the cervical spine

Two-layer collagen membrane for filling and repairing bone defects

Powder and solvent for the preparation of tissue adhesive in the amount and assortment specified in Annex 5 to the SWZ

A set of elements for the external stabilizer in the number and assortment specified in Annex 5 to the SWZ
