1.The subject of the contract is the purchase, supply, commissioning of medical devices with equipment and conducting training of employees in the use of this equipment. The medical equipment offered by the Contractor must have current permits for use in Poland – in accordance with the Act of 7.04.2022 on medical devices (Journal of Laws of 2022, item 974) and have a declaration of conformity (CE certificates). A detailed description of the subject of the contract is included in the Appendix. 1A – Appendix 1 G to SWZ. This description should be read with possible changes, e.g. as a result of the answers given.

2.The order is divided into 7 lots:

Part 1 – Anaesthesia machine 1 set

Part 2 – HD videocolonoscope 1 pc

Part 3 – Respirator 1 pc

Part 4 – Transport defibrillator 1 pc

Part 5 – Ultrasound machine with color Doppler 1 pc

Part 6 – Electrosurgical apparatus 1 pc

Part 7 – Operating table with equipment 1 set

The subject of the contract is the purchase and delivery of an anaesthesia device with a patient monitor (1 set). A detailed description of the subject of the contract is included in the Appendix. 1A to SWZ.

The subject of the order is the purchase and delivery of the HD Videocolonoscope (1 pc. A detailed description of the subject of the contract is included in the Appendix. 1B to SWZ.

The subject of the contract is the purchase and delivery of a respirator (1 pc). A detailed description of the subject of the contract is included in the Appendix. 1C to SWZ.

The subject of the contract is the purchase and delivery of a transport defibrillator (1 pc). A detailed description of the subject of the contract is included in the Appendix. 1D to SWZ.

The subject of the order is the purchase and delivery of an ultrasound machine with color Doppler (1 pc). A detailed description of the subject of the contract is included in the Appendix. 1E to SWZ.

The subject of the contract is the purchase and delivery of Electrosurgical Apparatus (1 pc). A detailed description of the subject of the contract is included in the Appendix. 1F to SWZ.
