I) Task 1: “Supply of liquid fuels based on fuel cards or other documents for vehicles of the Forest Transport Centre” within 24 months from the date of signing the contract. Projected quantity of supplies; ON - 1 280,000 L, Pb 95 — 15 450 L,, LPG — 12 500 L, Location: area of all Poland (minimum 3 stations in each province) and in selected destinations indicated in the offer form, at least one station or outside the border of that city, however, no further than 20 km.
II) Task 2: “Wholesale delivery of diesel fuel to service stations located in the Klenica Drzewny Klenica Street. Bolesława Chrobrego 76, 66 - 130 Bojadła and to Placówka Terenowa Lubsko Street Emilii Plater 15a, 68 - 300 Lubsko”. Expected quantity of supplies: ON - 200 000 L.
Task 1: “Supply of liquid fuels based on fuel cards or other documents for vehicles of the Forest Transport Centre”The subject of the order is the delivery of liquid fuels based on fuel cards or other documents for vehicles of the Forest Transport Centre, consisting of the provision of the Contractor's service stations for refuelling of the Contracting Authority's vehicles.
The main locations are localities; Świebodzin, Lubsko, Krosno Odrzańskie, Żary, Nowa Sól, Cybinka, Torzym, Sulechów.
Gas stations must comply with the requirements laid down by the rules for service stations in accordance with the Regulation of the Minister of Economy of 21 November 2005 on the technical conditions to be met by liquid fuel bases and stations, far-reaching transmission pipelines for the transport of oil oil and petroleum products and their location (i.e. Journal of Laws of 2014, item 1853, as amended).
Deadline and method of execution of the object of the order (delivery on the basis of fuel cards or other documents for OTL vehicles):
(a) The contract shall be carried out successively in the Contractor's service stations indicated by the Contractor in the localities referred to above.
b) After choosing the most favorable offer, the Contractor will be presented to the Contractor to sign the contract for the performance of the
(c) The settlement of the fuel sold to the Contracting Authority shall be carried out on the basis of Fuel Cards or other documents which will be the basis for mutual settlement and the issuance of VAT invoices.
(d) The form of the settlement issued to the Contracting Authority shall be described in detail in the draft agreement, Annex 7a.
Task 2: “Wholesale delivery of diesel fuel to service stations located in the Klenica Wood Plant and to the Lubsko Terenowa Placówka”Description: The subject of the order is the wholesale delivery of diesel fuel to the Placówka Drzewny in Klenica Street Bolesława Chrobrego 76, 66 - 130 Bojadła and to Placówka Terenowa Lubsko Street Emilii Plater 15a; 68 - 300 Lubsko

Method of execution of the order (autocystern delivery to the service stations for own needs in ZD Klenica and PT Lubsko):
(a) The order will be carried out successively with delivery to the Gas Station at the Drzewny Plant in Klenica, 76 Bolesława Chrobrego Street; 66 - 130 Bojadła, Lubusz Voivodeship and to the PT Lubsko field outpost: Emilii Plater 15a Street; 68 — 300 Lubsko (Lubsko Voivodeship).
b) After selecting the most favorable offer, the Contractor will be presented to the Contractor to sign the contract.
(c) Deliveries will be made on the next business day (Monday to Friday) from the date of the order notification, from 7:00 to 14:00.
(d) If the order is notified immediately before the statutory day off work, the delivery will be carried out on the next day after the day off work.
e) The order request will be followed by email or fax.
(f) The Contractor's acceptance of the order shall be confirmed by email or fax on the date of receipt of the order declaration from the Contracting Authority.
(g) Fuel should be supplied with autocysterns bearing a current certificate of authorisation for the carriage of fuels and complying with the requirements of the rules on the carriage of hazardous materials.
(h) The Contracting Authority shall inform that the settlement of the quantity of diesel delivered and invoicing shall be based on the issuance document at a reference temperature of 15 st C drawn up on the basis of the reference temperature of 15 st.

(i) The quantitative collection of the fuel will be made by the Contracting Authority after the fuel has been poured into the underground tank (with a capacity of 10m3 — ZD Klenica and 20m3 — PT Lubsko) by making jointly with
representative of the Contractor of measurement before flooding and 30 minutes after flooding with the help of legalized measuring equipment of the service station. From each delivery, the Ordering Party will take two samples
fuel for control purposes. A protocol for the receipt of fuels will be drawn up in accordance with the Annex to the 'model supply contract'.
(j) The Contractor shall provide, together with the subject of delivery, an up-to-date quality certificate issued by the manufacturer, certified by himself or the certificate of the accredited laboratory in
Polish language. The certificate or certificate should be linked to the proof of delivery (issue) by placing the number and date of the proof of delivery on the certificate/certificate. No quality certificate/certificate
or a photocopy of a certificate/certificate not assigned to a certain delivery will form the basis for the Ordering Party's refusal to accept the subject of delivery.
