Description:

1. The subject of the contract is the supply of licenses and software implementation, modernization of domain-specific systems, launch of public e-services and training along with the delivery of equipment for the City of Zamość, the Baranów Commune, the Biszcza Commune, the Kurów Commune and the Lubelski Poviat as part of a project co-financed from the Regional Program Operational Lubelskie Voivodship for 2014-2020, Priority Axis II Digital Lubelskie, Measure 2.1 Digital Lubelskie.

2. The contract is divided into 10 lots.

Lot 1 of the contract - Launch of public financial e-services for the City of Zamość

1. The subject of the contract in part 1 includes the launch of public financial e-services for the City of Zamość.

2. The order includes the following elements:

(1) Purchase of a tax portal license - 1 item,

(2) Implementation of the tax portal - 1 item,

(3) Software development of the domain system - 1 item,

(4) Implementation of an integrated domain system - 1 item,

(5) Development and implementation of 5PD e-services - 15 items

3. A detailed description of the subject of the contract is included in Annex 11a to the ToR and in the draft contract constituting Annex 10a to the ToR.

4. Place of implementation - the building of the City Hall in Zamość - Rynek Wielki 13, 22-400 Zamość.

5. The Contractor is obliged to perform the contract on the terms and conditions described in the ToR and annexes to the ToR.

6. The Awarding Entity informs that the trademarks, patents or origin, sources or specific processes indicated in the ToR and in the ToR, which characterize the product or service provided by a specific Contractor, are not intended to favor or eliminate certain Contractors or products. In accordance with art. 29 paragraph 3 of the Public Procurement Law Act, the Awarding Entity allows for an equivalent solution provided that they have inferior property and quality in relation to the one described. The Awarding Entity informs that in cases where it has indicated in the ToR and in the annexes to the ToR, a reference to standards, European technical assessments, approvals, technical specifications and technical reference systems referred to in art. 30 paragraph 1 point 2 and par. 3 of the Public Procurement Law allows for equivalent solutions to those described.

7. The Contractor, which refers to equivalent solutions described by the Employer, is required to demonstrate that the supplies or services offered by it meet the requirements specified by the Employer in the ToR and in the annexes to the ToR. The Purchaser will consider as equivalent the product only one that has the same or not worse quality and performance parameters in relation to the described

8. The contractor is obliged, in accordance with art. 29 paragraph 5 of the Public Procurement Law, to ensure that the implementation of the task takes into account the requirements in terms of accessibility for people with disabilities or design for all users.

9. Requirements for employment by the Contractor or subcontractor on the basis of an employment contract of persons performing the contract performance activities specified by the Employer, referred to in art. 29 paragraph 3a of the Public Procurement Law, the Awarding Entity concluded in point 26 of the ToR and in the draft contract constituting Annexes 10a to the ToR.

10. A condition of participation in the procedure for awarding the contract for lot 1 is a bid bond of PLN 15,000.00.

Lot 10 of orders - Supply of IT software and hardware for the Kurów commune

1. The subject of the order for lot 10 includes the supply of IT software and hardware for the Kurów Commune.

2. The order includes the following elements:

(1) Server room equipment - purchase of a server with an operating system - 1 item,

(2) Server room equipment - purchase of NAS device - 1 item,

(3) Server room equipment - purchase of UTM device - 1 item

3. A detailed description of the subject of the contract is included in Annex 11j to the ToR and in the draft contract constituting Annex 10j to the ToR.

4. Place of implementation - the building of the Commune Office in Kurów, ul. Lubelska 35, 24-170 Kurów.

5. The Contractor is obliged to perform the contract on the terms and conditions described in the ToR and annexes to the ToR.

6. The Awarding Entity informs that the trademarks, patents or origin, sources or specific processes indicated in the ToR and in the ToR, which characterize the product or service provided by a specific Contractor, are not intended to favor or eliminate certain Contractors or products. In accordance with art. 29 paragraph 3 of the Public Procurement Law Act, the Awarding Entity allows for an equivalent solution provided that they have inferior property and quality in relation to the one described. 9. A condition of participation in the procedure for part 10 of the contract is a bid bond of PLN 1,000.00.

Lot 2 - License delivery, implementation, expansion of IT systems to launch public e-services along with the purchase of training for the City of Zamość

1. The subject of the contract for lot 2 includes the delivery of licenses, implementation and expansion of IT systems to launch public e-services along with the purchase of training for the City of Zamość

2. The order includes the following elements:

(1) Purchase of a partner license for social dialogue and communication platform - 1 item,

(2) Implementation of the social dialogue and communication platform - 5 items,

(3) Development of the City's budget management system - 1 item,

(4) Implementation of the City's budget management system - 1 item,

(5) Purchase of the license of the communication system with organizational units - 112 items,

(6) Implementation of the communication system with organizational units - 56 items,

(7) Integration of the electronic document circulation system with the domain system - 1 item,

(8) Development and implementation of e-services for 3PD - 10 items,

(9) Conducting training in the functioning of e-services and ePUAP - 80 people,

(10) Conducting training in the field of environmental administration - 2 people.

1. The subject of the order for lot 3 includes the delivery of licenses, implementation and modernization of IT systems to launch public e-services along with the purchase of training for the Baranów commune.

2. The order includes the following elements:

(1) Purchase of a tax portal license - 1 item,

(2) Implementation of the tax portal - 1 item,

(3) Modernization of domain system software - 1 item,

(4) Implementation of an integrated domain system - 1 item,

(5) Purchase of the municipal budget management system license - 1 item,

(6) Implementation of the Commune budget management system - 1 item,

(7) Purchase of a license for the electronic documentation flow system - 1 item,

(8) Implementation of the electronic document circulation system - 1 item,

(9) Development and implementation of e-services for 5PD - 10 items,

(10) Development and implementation of e-services for 3PD - 8 items,

(11) Training on the functioning of e-services and ePUAP - 20 people,

(12) Conducting training in the field of environmental administration - 1 person.

4. Place of implementation - the building of the Commune Office in Baranów, ul. Rynek 14, 24-105 Baranów.

1. The subject of the order for lot 4 includes the delivery of licenses, implementation and modernization of IT systems to launch public e-services along with the purchase of training for the Commune of Biszcza.

2. The order includes the following elements:

(1) Purchase of a tax portal license - 1 item,

(2) Implementation of the tax portal - 1 item,

(3) Modernization of domain system software - 1 item,

(4) Implementation of an integrated domain system - 1 item,

(5) Purchase of the municipal budget management system license - 1 item,

5. The Contractor is obliged to perform the contract on the terms and conditions described in the ToR and annexes to the ToR.

1. The subject of the order for lot 6 includes the launch of public e-services at maturity level 3 for the Lublin Poviat.

2. The contract includes the development and implementation of e-services for 3PD - 16 items

3. A detailed description of the subject of the contract is included in Annex 11f to the ToR and in the draft contract constituting Annex 10f to the ToR.

4. Place of implementation - the building of the Poviat Starosty Office in Lublin, ul. Spokojna 9, 20-074 Lublin.

5. The Contractor is obliged to perform the contract on the terms and conditions described in the ToR and annexes to the ToR.

The Awarding Entity informs that the trademarks, patents or origin, sources or specific processes indicated in the ToR and in the ToR, which characterize the product or service provided by a specific Contractor, are not intended to favor or eliminate certain Contractors or products. 10. A condition of participation in the procedure for part 6 of the contract is a bid bond of PLN 1,500.00.

Lot 7 of orders - Supply of IT software and hardware for the City of Zamość

1. The subject of the order for lot 7 includes the supply of IT software and hardware for the City of Zamość

2. The order includes the following elements:

(1) Administrator's station equipment - purchase of a computer set with an operating system - 1 item,

(2) Administrator's station equipment - purchase of a portable computer with an operating system - 1 item,

(3) Server room equipment - purchase of disk array - 1 item,

(4) Server room equipment - purchase of a server with an operating system - 2 items,

(5) Server room equipment - purchase of server access licenses - 50 items,

(6) Server room equipment - purchase of virtualization and backup software licenses - 2 items,

(7) Server room equipment - purchase of UPS - 1 item,

(8) Server room equipment - purchase of UTM device - 1 item

(1) Server room equipment - purchase of a server with an operating system - 1 item,

(2) Server room equipment - purchase of UPS batteries - 2 pcs,

(3) Server room equipment - purchase of NAS device - 1 item,

(4) Server room equipment - purchase of a network switch - 2 items,

(5) Server room equipment - purchase of UTM device - 1 item

(1) Workplace equipment - purchase of computer sets with an operating system - 5 items,

(2) Server room equipment - purchase of a server with an operating system - 1 item,

(3) Server room equipment - purchase of a central UPS - 1 item,

(4) Server room equipment - purchase of UTM device - 1 item

8. The contractor is obliged, in accordance with art. 29 paragraph 5 of the Public Procurement Law, to ensure that the implementation of the task takes into account the requirements in terms of accessibility for people with disabilities or design for all users.

9. A condition of participation in the procedure for part 9 of the contract is a bid in the amount of PLN 2,000.00.
