1. The subject of the contract is "Increasing the effectiveness of medical rehabilitation services by equipping the SPZZOZ in Staszów with a rehabilitation robot for gait reeducation".
2. A detailed description of the subject matter of the contract is contained in this Terms of Reference and:
- Appendix 2 – Required technical parameters.
- Appendix 3 – Draft Agreement.
1. The subject of the contract is "Increasing the effectiveness of medical rehabilitation services by equipping the SPZZOZ in Staszów with a rehabilitation robot for gait reeducation".
2. The scope of the contract includes delivery, assembly along with the required installations (possible adaptation of the room, if necessary) along with commissioning
and training of the Contracting Authority's employees.
3. The goods offered by the Contractor must:
a) Possess valid documents allowing for admission to trading in the territory of the Republic of Poland in accordance with the provisions of the Act of 7 April 2022
on medical devices and the method of classification on the basis of the Regulation of the Minister of Health
of 5 November 2010 on the classification of medical devices, if applicable.
b) meet the requirements specified by the Contracting Authority in the Terms of Reference,
In particular, the conditions specified in the description of the subject of the contract - attachments with the required technical parameters.
c) be brand new, fully functional, not regenerated, not demo version.
d) be manufactured at least in 2023,
(e) comply with the quality and technical standards resulting from the function and intended use,
(f) be free from defects in material, construction, physical and legality,
g) may not be encumbered with any rights in favour of third parties.
4. The Offered Order Item must be complete with all subassemblies, parts and
and materials necessary for commissioning and use. The offered subject of the contract must meet the requirements of the Ordering Party.
5. The description of the offered product should not raise any doubts of the Ordering Party. The description should indicate that the offered subject of the contract is of the same parameters as required by the Ordering Party or better, by precisely indicating the parameters of the offered equipment.
