1. The subject of the order includes:
1) Delivery, installation and commissioning of a complete, factory new computed tomograph, larger than 32-row, included with medical stations, together with all components of the tomograph, its equipment, accessories, auxiliary and collaborative devices, and software, purchase of relevant licenses and retrofitting into additional network equipment in accordance with Annex 2 to SWZ — a detailed description of the object of the order.
2) Adaptation and installation of the supplied system in the computed tomography studio and adapting it to the applicable requirements in this regard (permanent cover plans, adaptive tests and obtaining permits for the work of the studio). Adaptation — according to the scope of adaptive work set out in Annex 2 to SWZ — a detailed description of the subject matter of the contract.

2) Adaptation and installation of the supplied system in the computed tomography studio and adapting it to the applicable requirements in this regard (permanent cover plans, adaptive tests and obtaining permits for the work of the studio). Adaptation — according to the scope of adaptive work set out in Annex 2 to SWZ — a detailed description of the subject matter of the contract.
2. The Contracting Authority shall enable the Contractors to carry out a local vision in order to familiarize themselves with the place of implementation and to assess the conditions associated with the performance of the work being the subject of the contract. Vision may be carried out by prior agreement with the Ordering Party.
3. Annex 2 to SWZ — a detailed description of the object of the contract containing a schedule of technical parameters must be completed by the Contractor and attached to the tender. The parameters given in the table are the minimum limit requirements (cut-off), the failure of which will result in the rejection of the offer. No entry in the “Offered Parameters” box will be considered as failure to comply with the parameter resulting in the rejection of the offer.
4. Equipment offered should be CE certified — must be provided at the signing stage of the contract.
5. Instructions for the operation of the devices must be made in English, the print in 1 ex. supplied with the device.
6. In the case of doubts of the Contracting Authority regarding the fulfilment of technical requirements - set out in the TECHNICAL PARAMETERS table, the Ordering Party reserves the right to request a presentation of the offered product for verification, inter alia by asking the Contractor for a presentation offered equipment.
7. Delivery of the object of the order will be carried out by transportation of the Contractor and at his expense, along with unloading at the Hospital in Buy, Karola Miarki 14 Street, 46-082 Buy.
8. As part of the offer, the Contractor is obliged to provide and install equipment in the specified room of the Ordering Party. All costs associated with the supply and installation of the supplied device shall be borne by the Contractor.
9. The device shall be accompanied by a technical passport with an up-to-date technical test confirming the performance of the device carried out by the authorised service.
10. The Contracting Authority allows the contractor to entrust the execution of part of the contract to subcontractors (with whom he has entered into a subcontract as defined in Article 7 (27) pzp). In this case, the contractor shall be obliged to indicate in his tender the parts of the contract (scope) which the execution of which he intends to entrust to the subcontractor and to provide the full names and data of the subcontractors by the contractor. If the subcontractor is entrusted to the execution of the contract, the Contractor shall be liable for the actions and omissions of such an entity as his own. The Contractor is obliged to indicate in JEDZ in Part II Section D the parts of the contract whose execution he intends to entrust to subcontractors and specify the subcontractor companies if they are already known.
