Owing to capacity issues with the current provider, the States of Jersey are seeking a provider of an on and off island orthopaedic spinal surgery service. Currently, the majority of patients with low back pain are referred to the Back Assessment Clinic, which is a multi-disciplinary service run as part of the Pain Department. The Back Assessment service receive about 1 200 referrals per year.
The majority of referrals received by the Orthopaedic team from BAC are therefore patients who have undergone extensive investigations/treatments/education and some minor procedures and for whom a surgical assessment is considered to be the most appropriate next step. Patients with other spine pathology are referred to the orthopaedic service directly.

Currently uninstrumented spinal decompressions are performed by the team in Jersey. More complex cases including tumours/fractures and patients for instrumented fusions are referred off island. Occasionally complex cases are undertaken on island where special circumstances determine that it would be inappropriate for the patient to travel.
Approximately 10-12 medical evacuations off-island take place each year for spinal emergencies — cord compression, severe spinal stenosis and cauda equina syndrome.
All adult and paediatric scoliosis surgery is undertaken off island.
There are presently 3 providers of off island complex spinal surgery. The States of Jersey aim would be to consolidate all of the spinal surgery activity to 1 main provider, assuming there is sufficient capacity. Providers can, however, bid for all or parts of the service as defined below:
1. On island visiting consultant outpatient and spinal surgery operating service;
2. Off island Urgent non elective/non elective/non emergency medical evacuation service for spinal emergencies;
3. Off island Elective complex spinal surgery;
4. Off island surgery and on island outpatients: Adult and paediatric scoliosis surgery.
