1. Delivery to the Ordering Party's headquarters - (including transport and unloading, assembly and commissioning) of two mobile robots - in accordance with the set of technical parameters specified in Annexes 2.1. and 2.2. to the SWZ.
2. Installation of the subject of the contract and training of employees in the operation, operation and maintenance of the subject of the contract.
3. Delivery of the subject of the contract will take place by transport of the Contractor and at his expense, along with unloading to the Hospital in the Room, Namysłowska 22, 46-034 Room.
Mobile upper body rehabilitation robot1. Delivery to the Ordering Party's headquarters - (including transport and unloading, assembly and commissioning) of a mobile rehabilitation robot of the upper body parts - in accordance with the set of technical parameters specified in Annex 2.1. to the SWZ.
2. Installation of the subject of the contract and training of employees in the operation, operation and maintenance of the subject of the contract.
3. All activities and works related to the assembly, placement and installation of the subject of the contract offered by the Contractor, necessary for the proper and consistent functioning of the subject of the contract, the Contractor shall be obliged to include in the price of the offer.
4. The offered subject of the contract must meet the requirements set out in the SWZ, it must be brand new, unconditioned, non-exhibition, in a complete condition, i.e. guaranteeing its launch and use as intended without making additional purchases of elements and accessories.
5. The table with the required parameters (Annexes No. 2 to the SWZ, list of technical parameters) must be completed by the Contractor and attached to the offer. The parameters given in the table are the minimum limit (cut-off) requirements, failure to meet which will result in rejection of the offer. Lack of entry in the "Offered parameters" field will be treated as failure to meet the parameter resulting in rejection of the offer.
6. The offered subject of the contract should have a CE certificate – it should be delivered at the stage of signing the contract.
7. Instructions for use of the subject of the order must be prepared in Polish, printout in 1 copy attached to each assortment and delivered with the assortment.
8. Delivery of the subject of the contract will be carried out by the Contractor's transport and at his expense, along with unloading to the Hospital in the Room, Namysłowska 22, 46-034 Room.
9. As part of the offer, the Contractor is obliged to deliver and install the subject of the contract in the indicated room of the Ordering Party and to collect transport packaging after the installed equipment and disposal on its own and at its own expense. All costs related to the delivery, installation and calibration of the delivered devices shall be borne by the Contractor.
10. The warranty includes free repairs, maintenance, inspections along with materials and spare parts. Free, periodic warranty inspections according to the manufacturer's recommendations.
11. Each subject of the contract must be accompanied by a technical passport. The Contractor is obliged to provide the information necessary to complete the technical passport of the device.
12. If it is necessary to repair the equipment within the warranty period with an expected time longer than 30 days, the Contractor will be obliged to provide the Ordering Party with replacement equipment with no worse parameters, at its own expense and risk.
