The subject of the contract is the service of collection, loading, transport and neutralization of medical waste for the Provincial Hospital Center of the Jelenia Góra Valley and non-local units.
2. Loading, collection and transport of hazardous medical waste from:
Ogińskiego 6, 58-506 Jelenia Góra
b) Sanatoryjna 27, 58-530 Kowary
c) Health Care Facility Wysokie górska 19, 55-420 Bolków
3. Waste collection shall be carried out:
From the registered office in Jelenia Góra for the code 180102 on Mondays, Wednesdays and Fridays of each week, for codes 180103, 180106, 180108 180109 on Thursday of each week from 7.00 to 12.00.
In non-local units, the reception for the code will 180103 take place-
Sanatoryjna 27, 58-530 Kowary once a week – a day to be agreed – hours in accordance with the assessment criterion in point 15 paragraph 1 SWZ.
Wysokie górska 19, 55-420 Bolków once every two weeks – every second Wednesday – hours in accordance with the assessment criterion in point 15 paragraph 1 SWZ.
4. The waste taker, transporter and waste disposer is obliged to make current entries in the BDO register within the scope of the Waste Transfer Card issued by the Ordering Party, hereinafter referred to as "KPO", "Completed takeover"
and "Confirmed Transport". If it is necessary to withdraw or reject the KPO, the Contractor is obliged to notify the Contracting Authority of this fact by phone and confirm the information to the e-mail address indicated by the Contracting Entity.
5. The contract concerns waste with codes for the period of:
(a) 18 01 02-Body parts and organs and blood containers and blood preservatives
storage / 24 months
b) 18 01 03 – Other wastes containing live pathogenic micro-organisms or their toxins/ 24 months
c) 18 01 06 - Chemicals including chemical reagents / 24 months
d) 18 01 08 – Cytostatic and cytotoxic drugs/ 24 months
e) 18 01 09 - Other medicines not mentioned in 18 01 08/ 24 months
. The service should be provided in accordance with the "principle of proximity" described in accordance with the Act of 14 December 2012 on waste (consolidated text, Journal of Laws of 2022, item 699, as amended). In accordance with the principle of proximity, it is forbidden to dispose of infectious medical waste and infectious veterinary waste outside the province in which it was generated. It is allowed to dispose of infectious medical waste and infectious veterinary waste in the area of a province other than the one on which it was generated, in the nearest installation, in the absence of installations for the disposal of this waste in the area of a given province or if the existing installations do not have free processing capacity. In addition, the requirements of Article 95(2) and (3) of the Waste Act, which requires infectious medical waste to be disposed of by thermal treatment in hazardous waste incineration plants, should be taken into account and at the same time prohibits the disposal of medical waste in waste co-incineration plants.
