The South Eastern Health and Social Care Trust (SET), as Lead Partner in the ‘Transitions’ project, requires a Contractor(s) (or consortium) to deliver the identified intervention support programmes across the South Eastern SIF Zone.

SET will manage the project on behalf of the Office of First Minister and Deputy First Minister's (OFMDFM) Southern SIF Zone.

The delivery will take place in 4 defined areas, North Down and Ards, Down, Lisburn and Colin.

This commission is being procured on a Lots basis covering 4 separate geographic areas as above. Tenders can bid for any or all Lots.
