1. The subject of the contract is the provision of services in the field of collecting municipal waste from the owners of inhabited real estate, summer houses and other real estate used for recreational purposes, from the area of the Municipality of Sułkowice. 2. The subject of the contract is described in detail in the Description of the Subject of the Order (OPZ), which is an annex to this specification of the terms of the contract; 3. The Ordering Party shall require the employment of persons performing the following activities during the performance of the contract on the basis of an employment relationship by the Contractor or Subcontractor: driving vehicles collecting waste and operators loading waste. 4. During the performance of the contract, the Ordering Party is entitled to perform control activities towards the Contractor regarding the fulfillment by the Contractor or Subcontractor of the requirement of employment on the basis of the employment relationship of persons performing the activities indicated in point 3. The Ordering Party is entitled, in particular, to: − request statements and documents in the scope of confirming compliance with the above-mentioned requirements and assessing them, − request explanations in case of doubts regarding the confirmation of compliance with the above-mentioned requirements, − carry out on-site inspections of the performance of the service. 5. In the event of justified doubts as to the compliance with the labor law by the Contractor or Subcontractor, the Ordering Party may request an inspection by the National Labor Inspectorate. Supplementary contracts referred to in Article 214(1)(7): The contracting authority provides for the possibility of awarding, within 3 years from the date of awarding the basic contract to the existing service provider, a contract consisting in the repetition of similar services with a total value of up to 20% of the value of the basic contract. These orders include: • collection of municipal waste from owners of inhabited real estate, summer houses and other properties used for recreational purposes, from the area of the Sułkowice Municipality. • The contract will be awarded after the procedure provided for direct contracting, will be preceded by an appropriate invitation to negotiations and negotiations. Local Vision: The customer does not require an on-site inspection. A detailed description of the subject of the contract has been included in the OPZ, which is attached to the SWZ

The subject of this contract is the implementation of the municipal waste collection service arising in inhabited real estate, summer houses and other properties used for recreational purposes in the Municipality of Sułkowice. The scope of work includes: 1. Collection of municipal waste immediately from inhabited properties, summer houses and other properties used for recreational purposes, including: 1) non-segregated (mixed) municipal waste, 2) separately collected waste, i.e.: - paper and waste paper, - glass, - plastics, metal, textiles, multi-material packaging waste; - bio-waste, - ash and slag; 3) bulky waste and waste electrical and electronic equipment, 2. Collection at the Selective Waste Collection Point, i.e. PSZOK, of waste delivered on their own, by owners of inhabited real estate, summer houses and other properties used for recreational purposes from the Sułkowice Commune, taking into account the provisions of the Resolution of the City Council on the Regulations for maintaining order and cleanliness in the Sułkowice Municipality: 1) paper, 2) plastics, 3) metal, 4) glass, 5) multi-material packaging, 6) hazardous waste, 7) waste not eligible for household medical waste as a result of taking medicinal products in the form of injections and monitoring the level of substances in the blood, in particular needles and syringes, 8) used batteries and accumulators, 9) waste electrical and electronic equipment, 10) furniture and other bulky waste, 11) used tires, 12) construction and demolition waste, 13) textile and clothing waste, 14) bio-waste, 15) ash and slag, 3. Transfer of waste collected from inhabited properties, summer houses and other properties used for recreational purposes and municipal waste delivered directly by the owners of the above-mentioned property to the Selective Waste Collection Point, to the entity selected in the tender for the management of municipal waste along with transport from the Sułkowice Commune. The subject of the contract does not include the collection and management of municipal waste generated in connection with the conducted business activity (including agricultural activity).
