The service of collection, transport and neutralization/processing of hazardous infectious medical waste and hazardous medical waste and other than hazardous medical waste resulting from diagnosis, treatment and medical prophylaxis arising as a result of the Ordering Party's activity for a period of 12 months (guaranteed period).
The ordering parties are:
1) Szpital Wojewódzki w Opole sp. z o.o. (Lead Partner) and
2) Independent Public Health Care Centre Prof. Tadeusz Koszarowski Opole Oncology Centre in Opole (Partner).
After completion of the proceedings, each of the Contracting Parties shall individually sign a contract with the selected Contractor for the provision of the service of collection, transport and neutralization/processing of hazardous medical infectious waste and hazardous medical waste and non-hazardous medical waste resulting from medical diagnosis, treatment and prophylaxis.
The service of collection, transport and neutralization/processing of hazardous infectious medical waste and hazardous medical waste and other than hazardous medical waste resulting from diagnosis, treatment and medical prophylaxis arising as a result of the Ordering Party's activity for a period of 12 months (guaranteed period). As part of the concluded contract, the Contracting Authorities provide for the possibility of applying the right of option consisting in the possible extension of the contract with the Contractor for a further period of 12 months (option I), and another period of 12 months (option II) while maintaining the same conditions and scope of the subject of the contract as in the guaranteed contract. The maximum number of extensions is two, which means that the maximum duration of the contract in the case of exercising the option right can be a total of 36 months. A detailed description and scope of the subject of the contract is specified in the contract template (with attachments) constituting Annex No. 1 to the SWZ and in the PRICE FORM constituting Annex No. 2 to the SWZ, which will constitute an attachment to the concluded contracts.

2) Independent Public Health Care Centre Prof. Tadeusz Koszarowski Opole Oncology Centre in Opole - 66a Katowicka Street (Partner).
