Description:
Supply of reagents for allergy diagnosis.

1.Prem. zam. is before. and access to reactions. to the diagnostician. allergy using multi-parameter panel tests along with anal rental. dedicated to the execution of of these tests and reactions. to diagn. molecular allergy based on multi-parameter tests together with the rental of the office. necessary for the performance of the Bad. in the following groups:

Group 1.Supply of diagnostic reagents. allergy based on multi-parameter panel tests along with anal rental. dedicate. To be performed of those studies.

Group 2.Supply of reagents for molecular diagnostics of allergy (based on allergen components, allergen extracts and total IgE) along with the rental of equipment necessary to perform the test

2. The Contracting Authority provides for the application of the procedure referred to in Article 139(1) of the Public Procurement Law, i.e. the Contracting Authority shall first examine and evaluate the bids, and then shall qualify the Contractor whose bid has been rated the highest in terms of the lack of grounds for exclusion and the fulfilment of the conditions for participation in the post.

Group 1

Delivery of allergy diagnostic reagents based on multi-parameter panel tests along with the rental of an analyser dedicated to perform the above-mentioned tests.

Criteria for evaluation of bids

Within the scope of Group 1:

How offers are evaluated:

The criterion: price-index C-will be calculated according to the formula:

C=(Cn/ Cb)x60

Where:

Cn – the offer with the lowest gross price.

Cb-gross price of the examined offer.

B)Criterion: The deadline for the current order – indicator T – will be calculated according to the formula:

T= (To/ Tmax )x20

Where:

This is the number of points received by the bid being examined.

Tmax – the maximum number of points to be obtained under the criterion.

Number of points awarded under This:

1) Deadline for the current order – 5 business days-0 points.

2) Lead time for the current order – 4 working days – 20 points.

3) Lead time of the current order – 3 working days – 60 points.

4) Deadline for the current order – 2 working days – 100 points.

As part of the above-mentioned criterion, the contractor may receive a maximum of 100 points = 20%.

Attention:

Offering the date of the execution of the current contract by the contractor can only take place in full days. The Contractor may not propose deadlines, e.g. in hours or minutes or seconds or in total in days and other units

C.Criterion: The number of lines enabling the calculation of the calibration curve – indicator L – will be calculated according to the formula:

L=(Lp/Lmax)x20

Where:

Lp – the number of points received by the surveyed offer,

Lmax– the maximum number of points to receive within the criterion.

The contract does not apply to a project financed from EU funds.

Number of points awarded under Lp:

-3 lines for plotting the calibration curve – 0 pts.

-4 lines for plotting a calibration curve – 50 pts.

-5 or more lines for plotting a calibration curve–100 pts.

As part of the above-mentioned scheme, the contractor may receive a maximum of 100 points = 20%.

Evaluation of the offer – O-indicator will be calculated according to the formula:

O=C+T+L

Where:

O–evaluation of the offer.

C–price.

T – the date of completion of the current order.

L–number of lines to plot the calibration curve

Within the scope of Group 2:

How offers are evaluated:

C) Criterion: the ability to perform the test for a single patient without reagent losses – indicator B, will be calculated according to the formula:

B=(Bp/Bmax)x20

Where:

Bp– the number of points received by the audited offer,

Bmax – the maximum number of points to receive within the criterion.

Number of points awarded under Bp:

1) the possibility of performing the test for a single patient without reagent losses – 100 points.

2) inability to perform the test for a single patient without reagent losses – 0 points.

As part of the above-mentioned criterion, it can receive a maximum of 100 points = 20%.

D.Evaluation of the offer – indicator O-will be calculated according to the formula:

O=C+T+B

Where:

O–evaluation of the offer.

C–price.

T – the date of completion of the current order.

B-possibility of performing the test for a single patient without reagent losses

1. The Contracting Authority shall exclude Contractors on the basis of:

-Article 108(1) of the Public Procurement Law,

-Article 7(1) of the Act of 13 April 2022 on Special Solutions for Counteracting Support for Aggression against Ukraine and for the Protection of National Security,

- Article 5k of Council Regulation (EU) No 833/2014 of 31 July 2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine. EU No L 229, 31.7.2014, p. 1), hereinafter: Regulation 833/2014, as amended by Council Regulation (EU) 2022/576 amending Regulation (EU) No 833/2014 concerning restrictive measures in view of Russia's actions destabilising the situation in Ukraine (OJ L 229, p. 1). EU No L 111, 8.4.2022, p. 1), hereinafter: Regulation 2022/576.

2. The Contracting Authority shall additionally exclude the Contractor pursuant to Article 109(1)(4) of the Public Procurement Law.

The Contractor is excluded from the public procurement procedure pursuant to Article 108(1) of the Public Procurement Law.

The contractor shall exclude from the procurement procedure:

1) being an individual who was legally convicted of a crime:

(a) participation in an organised criminal group or union aiming to commit an offence or treasury offence referred to in Article 258 of the Criminal Code,

b) trafficking in human beings referred to in Article 189a of the Criminal Code,

c) referred to in Articles 228-230a, 250a of the Criminal Code, in Articles 46-48 of the Act of 25 June 2010 on Sport (Journal of Laws of 2020, item 1133, and of 2021, items 2054 and 2142) or in Article 54(1)-(4) of the Act of 12 May 2011 on the Reimbursement of Medicines, Foodstuffs for Special Nutritional Purposes and Medical Devices (Journal of Laws of 2022, item 463, 583 and 974 ),

d) financing a terrorist offence referred to in Article 165a of the Criminal Code, or the offence of preventing or hindering the determination of the criminal origin of money or concealing its origin, referred to in Article 299 of the Criminal Code,

e) of a terrorist nature referred to in Article 115§20 of the Criminal Code, or aimed at committing this crime,

f) entrusting the performance of work to a minor foreigner referred to in Article 9(2) of the Act of 15 June 2012 on the consequences of entrusting work to foreigners residing in the territory of the Republic of Poland contrary to the regulations (Journal of Laws of 2021, item 1745),

g) against economic transactions referred to in Articles 296-307 of the Criminal Code, an offence of fraud referred to in Article 286 of the Criminal Code, an offence against the credibility of documents referred to in Articles 270-277d of the Criminal Code, or a fiscal offence,

h) referred to in Article 9(1) and (3) or Article 10 of the Act of 15 June 2012 on the Consequences of Entrusting Work to Foreigners Staying in the Territory of the Republic of Poland in Violation of the Regulations or for a relevant prohibited act specified in the provisions of foreign law;

2) if an incumbent member of its management or supervisory body, a partner in a general partnership or partnership, or a general partner in a limited partnership or a limited joint-stock partnership or a commercial proxy has been finally convicted of an offence referred to in point 1,
