1. The subject of the contract is a service in the field of collection, transport and disposal of medical waste in accordance with the price form constituting Annex 2 to the SWZ and in accordance with Annex 4 to the SWZ.
2. The Contracting Authority informs that in the proceedings in question conducted under the unlimited tender procedure, it provides for the possibility of applying the procedure referred to in Article 139(1) of the Public Procurement Law. The Contracting Authority, using the aforementioned procedure, will first examine and evaluate the tenders, and then make a subjective qualification of the Contractor whose offer has been rated the highest in terms of the lack of grounds for exclusion and meeting the conditions for participation in the procedure.

The subject of the contract is to determine the rules on which the Contractor will provide services to the Contracting Authority in the field of collection, transport and disposal of medical waste from the activities of the Contracting Entity, the type of which together with the codes (in accordance with the waste catalog, constituting an attachment to the Regulation of the Minister of Climate of 2 January 2020 on waste catalogs, Journal of Laws of 2020, pos.10)
2. The estimated amount of medical waste to be collected, transported and disposed of in the entire term of the contract is 380,000 kg and depends on the needs of the Ordering Party resulting from its activities
3. In the offer form, the Contractor should specify the place where the waste will be disposed of.
