The subject of the contract is the provision of specialized care services. The order was divided into two Parts. The Contractor may submit a tender for any Part of the contract (for two Parts or for one Part of the contract). Lot 1 of the contract, the subject of which is the provision of specialised care services for persons with aphasia. Lot 2 of the contract for the provision of specialised care services for children and for adults on the autism spectrum, including those with Asperger's syndrome
Detailed information on the subject of the contract and the manner of its implementation is specified in Annex 1 to the SWZ - Gdynia Standard of Specialized Care Services Provided at the Place of Residence, hereinafter referred to as the Standard. The description of the subject of the contract for Part 1 and Part 2 is specified in swz, including the model contract, which constitutes Annex No. 2.1 for Part No. 1 and Annex No. 2.2 for Part No. 2 to swZ.
Part No 1 of the orderLot 1 of the procurement
1) The subject of the contract is the provision of specialized care services for people with aphasia from the area of Gdynia, hereinafter referred to as "The Service", specified in the Act of 12 March 2004 on social assistance and the Regulation of the Minister of Social Policy of 22 September 2005 on specialized care services. The scope of the Service includes teaching and developing the skills necessary for independent living and preparation for specialist therapeutic support.
2) The service will be provided to 20 people, in the total amount of 1200 hours during the term of the contract (i.e. an average of 60 hours per person). Persons for whom the Service will be provided will be indicated by the Ordering Party. The service will be provided on working days from Monday to Friday from 7:00 to 19:00 (it is permissible to provide services on Saturday, provided that all interested persons agree to it).
Remark! The Ordering Party points out that the term of the hours of provision of services should be understood only as the actual time of provision of services. The Ordering Party means a clock hour, i.e. 60 minutes.
3) The Ordering Party reserves the right to reduce the expected number of hours of services provided, in particular depending on the number of persons applying for the indicated form of assistance or incomplete securing of funds for the provision of services by the Pomeranian Voivode. The number of hours of services given above is the maximum, while the minimum number of hours of services provided during the period of execution of the subject of the contract is 600 hours. The Contractor is not entitled to claim the order of the full number of hours. In the case of ordering an incomplete number of hours, the Contractor will be entitled only to remuneration resulting from the services provided and he will not submit claims regarding the implementation of the remaining part.
4) Detailed information on the subject of the contract and the manner of its implementation is specified in Annex 1 to the SWZ - Gdynia standard of specialized care services provided at the place of residence", hereinafter referred to as the Standard. A detailed description of the subject of the contract is specified in the SWZ.
5) The individualized scope of services for each person covered by them will be determined each time by the Ordering Party through administrative proceedings and provided in writing as part of the concluded contract with the selected Contractor.
6) The Contractor will be obliged to provide high-quality care services.
1. The Ordering Party does not reserve the obligation of the Contractor to perform key parts of the contract personally.
2. A Contractor who invokes equivalent solutions is obliged to demonstrate that the solution offered by him meets the requirements specified by the Ordering Party.
In this case, the Contractor shall attach to the tender a list of equivalent solutions with its description or standards.
3. The Ordering Party does not provide for the obligation for the Contractor to carry out a local inspection and to check by the Contractor the documents necessary for the performance of the contract available on the spot at the Ordering Party.
4. The Contracting Authority does not allow the possibility of submitting a variant offer referred to in Article 92 of the PPL Act, does not provide for the conclusion of a framework agreement and the establishment of a dynamic purchasing system and does not provide for the selection of the most advantageous offer using an electronic auction, does not provide for the award of contracts pursuant to Article 214(1)(7) of the PPL Act.
5. The Ordering Party shall not allow the submission of tenders in the form of electronic catalogues or the attachment of electronic catalogues to the offer in the situations specified in Article 93 of the PPL Act.
6. Settlement of the order is made in PLN.
7. The Contracting Authority does not provide for reimbursement of the costs of participation in the procedure, does not provide for the granting of advances for the performance of the contract.
Part 2 of the orderLot 2 of orders
1) The subject of the contract is the provision of specialized care services for children and for adults on the autism spectrum, including those with Asperger's syndrome from Gdynia, hereinafter referred to as "Services", specified in the Act of 12 March 2004 on social assistance and the Regulation of the Minister of Social Policy of 22 September 2005 on specialized care services.
2) The services will be provided in the amount of 16 200 hours during the term of the contract (on average 1350 hours per month), to 45 people per month, indicated by the Ordering Party, on working days from Monday to Friday from 7:00 to 19:00. It is permissible to provide the Services on Saturday, provided that both parties agree to it.
Remark! The Ordering Party points out that the term of the hours of provision of services should be understood only as the actual time of provision of services. The Ordering Party means a clock hour, i.e. 60 minutes.

The Contractor is not entitled to claim the maximum number of hours. In the case of ordering an incomplete number of hours, the Contractor will be entitled only to remuneration resulting from the Services provided and he will not make claims regarding the implementation of the remaining part.
4) Detailed information on the subject of the contract and the manner of its implementation is specified in Annex 1 to the SWZ - Gdynia standard of specialized care services provided at the place of residence, hereinafter referred to as the Standard. A detailed description of the subject of the contract is specified in the SWZ.
5) The Contractor will be obliged to dispose of premises in Gdynia for the purposes of coordination, settlement and contact in matters of services provided. The premises should have a direct telephone number, access to e-mail and be available to customers at least between 8:00 and 16:00 on working days.
