Other conditions regarding the performance of the contract are specified in the SWZ and in Annex 3 to the SWZ.

2. The complete offer consists of (obligatory):

1. The offer is prepared in accordance with the template of Annex 1 to the SWZ. Offer form;

2. Completed Attachment No. 1A. Price calculator in accordance with the formula constituting Annex 1A to the SWZ.

3. Declaration in the form of a European Single Procurement Document, according to Annex 2 to the SWZ;

4. Statement, according to Annex 7 to the SWZ – if applicable;

5. Statement on the lack of grounds for exclusion according to Annex 8 to the SWZ.

6. Statement, according to Annex 9 to the SWZ – if applicable;

7. In the case of lodging a bid bond in a non-monetary form, the original document confirming the lodging of the bid bond in electronic form signed with a qualified electronic signature.

8. Power of attorney or other document confirming the authorization to represent the contractor/contractors jointly applying for the award of a public contract / entity providing resources on the principles set out in Article 118 of the Act – if applicable;
