Delivery of surgical staplers and loads for staplers, retractors, clips and consumables for surgical laser for the needs of CPiT in Bystra

The subject of the contract is the supply of surgical staplers, loads for staplers, retractors, clips and consumables for surgical laser for the needs of the Center of Pulmonology and Thoracic Surgery in Bystra, in the assortment and quantities specified in detail in Annex 2 to the SWZ - assortment and price form. The subject of the order has been divided into 5 packages. The Ordering Party allows partial bids for individual Packages. Each contractor has the right to submit an offer for any number of Packages selected by them, provided that the offer must contain a valuation of the entire assortment within the Packages for which the Contractor submits an offer. The Ordering Party requires that the expiry date of the offered products be not shorter than 6 months from the moment of delivery of the order to the Ordering Party.

Package No. 1 - Staplers and loads for staplers, wound retractors

The subject of the contract in the scope of this part is the supply of staplers and loads for staplers, wound retractors, described in detail in the scope of assortment and quantities indicated in Annex 2 – assortment and price form. Under the term delivery, the Ordering Party shall understand cyclical deliveries carried out during the term of the contract, in accordance with orders placed by the Ordering Party, adapted to its current needs. The Ordering Party requires that the expiry date of the offered products be not shorter than 6 months from the moment of delivery of the order to the Ordering Party.

Order. before selecting the most advantageous offer, call the Contractor whose offer was rated the highest to submit within the prescribed period, not shorter than 10 days from the date of the call, current as at the date of submission of subjective evidence indicated in Chapter II, paragraph 6 SWZ - II. Documents submitted on call for the purpose of potw. no grounds for exclusion: Inform. from the National Court Register within the scope of Article 108(1)(1), (2) and (4) of the Public Procurement Law, Statement. The Contractor, within the scope of Article 108(1)(5) of the Public Procurement Law drawn up not earlier than 6 months before its submission, does not belong. to the same capital group.- Appendix No. 5 to SWZ, Oświad. Under the term delivery, the Ordering Party shall understand cyclical deliveries carried out during the term of the contract, in accordance with orders placed by the Ordering Party, adapted to its current needs. The Ordering Party requires that the expiry date of the offered products be not shorter than 6 months from the moment of delivery of the order to the Ordering Party.
