The order is co-financed from the budget of the Malopolska Region – participation in the implementation of the investment task entitled: Purchase of a robot for spine surgery for the Neurosurgery Clinic at the St. Luke Provincial Hospital SP ZOZ in Tarnów to the contract No. II/251/PS/2667/22 of 28.07.2022. The subject of the contract includes the delivery to the Ordering Party's headquarters of medical equipment, the required technical parameters of which are specified in Appendix No. 1B to this specification, as well as the assembly and commissioning of the delivered equipment and training of the Ordering Party's employees at the Ordering Party's headquarters in the field of equipment operation, provision of guaranteed maintenance services in the period for which the warranty for the offered subject of the contract is granted.

The Ordering Party requires that the offered product meets the requirements described in the Act on Medical Devices of May 9, 2022 (Journal of Laws of 2022, item 974).

. Deadline for the performance of the contract:

The Contractor undertakes:

a) deliver, install and launch brand new equipment by 01 December 2022

b) train the personnel indicated by the Ordering Party in accordance with the conditions and deadlines indicated in Annex 1B Warranty and Service Conditions to SWZ.
