Lambeth Clinical Commissioning Group (CCG) are procuring an integrated community-based diabetes service to be delivered via a multi-disciplinary team which is evidence based and cost effective and supports patients with type 2 diabetes to achieve better health outcomes.

Lambeth Clinical Commissioning Group (CCG) are procuring a community diabetes service consisting of the following elements:

• referral management;

• advice and guidance for patients and healthcare professionals;

• community clinics for type 2 diabetes care;

• self management support and structured education provision for patients at high risk of developing diabetes, newly diagnosed or with pre-existing diabetes;

• virtual clinics in primary care;

• education and learning for primary and community care.
