Description:
Reception from the site of Przedsiębiorstwo UServices Komunne sp. z o.o. in Tarnów large waste with code 20 03 07 together with their development in 2022

1. The minimum quantity of waste (minimum scope of the order), i.e. the amount of waste the Contractor is obliged to collect from the Contracting Authority as part of the execution of the order is 1000 (Mg).

2. The maximum quantity of waste (the maximum possible scope of the contract), i.e. the amount of waste which the Contractor is obliged to collect from the Contracting Authority in the course of the execution of the order if the Ordering Party exercises the right of the option, in the situation described in paragraph 3.3, is up to 3500 (Mg).

1. The following conditions for the provision by the Contractor of services covered by the subject of the contract shall be fixed

1) Reception of waste will be carried out by exporting them from the Contracting Authority's territory at 31 Komunalna Street, 33-100 Tarnów (place of collection of waste) at the cost and risk of the Contractor, by the Contractor's own transport, i.e. vehicles adapted to the transport of waste with code 20 03 07.

2) Reception of waste by the Contractor from the place of receipt will be carried out on the working days of the Ordering Party, i.e. from Monday to Friday from 7-14 hours, except on the statutory days free of work, on the basis of the call referred to in paragraph 3.

3) Each order for the receipt of waste from the place of their receipt, the Ordering Party shall make in a call sent by e-mail to the Contractor's e-mail address or notified by telephone at the Contractor's phone number with simultaneous confirmation of the call via e-mail to e -mail Contractors. In the call, the Contracting Authority shall specify each time the quantity (Mg) of waste to be collected by the Contractor from the place of collection of waste.

4) Reception of waste by the Contractor from the place of receipt in the quantity (Mg) indicated in the call referred to in point 3 shall be within 3 working days from the date of submission to the Contractor by the Contractor in question.

5) Loading of waste at the place of collection on the Contractor's vehicles lies on the Contractor's side.

6) The implementation of the confirmation of the receipt of waste will be carried out through the electronic Product and Packaging Database and Waste Management (hereinafter BDO).

7) The actual mass of waste [Mg] intended for receipt by the Contractor shall be determined each time by means of a legalised car scale located at the premises of the Contracting Authority (place of waste collection) and confirmed by evidence of weighing from a computer program operating it weight system. After the waste is weighed, the Contracting Authority in BDO will create and approve a KPO in which it will indicate the quantity (Mg) of waste for collection by the Contractor consistent with the proof of weighing. The KPO, together with the proof of weighing, the Contracting Authority shall transfer to the driver.

8) The Contractor shall, immediately after receiving the waste from the place of collection, verify that the waste is in accordance with the code provided by the Contracting Authority in the CPO.

9) The total mass of waste [Mg] received by the Contractor during the given month of service provision will be confirmed by the Contractor immediately after the end of that month in the balance sheet report of the mass of waste received by the Contractor on the basis of data from that period resulting from BDO, and sent to the address of the Ordering Party's email.

10) The Contractor is obliged to transport and use waste received from the Contracting Authority in accordance with the provisions of law and recovery processes resulting from the provisions of the Act of 14 December 2012 on Waste. At the time of transfer (collection) of waste from the Contracting Authority, the Contractor shall assume full responsibility for the waste seized, including due conduct and for any legal effect resulting from it and for further development in accordance with the requirements of the Act of 14 December 2014 on waste. The Contractor shall be obliged to provide the Contractor, without a call, documents confirming the manner and place of waste received from the Contracting Authority in the previous month at the latest by the 5th working day of each following month.

2. The Contractor must comply with all applicable formal and legal requirements related to the execution of the subject of the contract. In particular, it must hold current contracts at all times:

1) entry in the Register of entities introducing products, packaging products and waste management in the field of waste transport code 20 03 07, as required by the Waste Act of 14 December 2012 (BDO).

2. An authorisation issued by the competent authority covering the conduct of waste collection or processing activities of code 20 03 07,
