1. The subject of the contract is the purchase and supply of medical devices, namely centrifuges for the treatment of whole blood in the amount of 3 pcs.

The Contractor shall deliver, unpack and deploy the equipment with its own forces and perform installation and operational qualification in accordance with the requirements of the Contracting Authority and train the staff in the use of the delivered devices within max. 40 calendar days from the date of signature of the contract
4. The Contractor shall bear all costs for causing damage caused by improper packaging or transport to the place of installation.
5. During the warranty period, the Contractor shall carry out inspections of the equipment at least once a year and after each critical failure of the device at its own expense.
6. During the warranty period, the Contractor shall provide, at its own expense, repairs, validations (replacement of each of the elements, subassemblies or assemblies of delivered devices that have been damaged due to causes and defects in construction, production or material) and validation of devices after repairs.
7. The Contractor shall provide an authorized service located in the Polish.
8. The Contractor shall provide post-warranty service for a period of min. 10 years.
9. The Contractor shall attach the instruction manual of the devices in Polish together with the instruction
maintenance and cleaning.
10. The Contractor shall ensure automatic data transmission from the devices to the database management program BLOOD BANK used in RCKiK Rzeszów. The Contractor shall include in the price of the offer the costs of adapting the transmission program to the program of the Contracting Authority – Bank of Blood.
In the event of a change in the IT system in the Blood Service during the term of the contract, the Contractor shall ensure at its own expense the adaptation of two-way data transmission to the new system.
11. Warranty period for devices min. 24 months from the date of commissioning.
12. Time to intervene to report a failure max 72 hours.
