The subject of the contract is the provision of specialized care services for adults with mental disorders from Gdynia, hereinafter referred to in the Act of 12 March 2004 on social assistance and the Regulation of the Minister of Social Policy dated 22 September 2005 on specialist care services, in the following areas of operation (in Sc). Services will be provided in a maximum dimension of 99,600 hours during the duration of the contract, to approximately 125 persons per month, indicated by the Ordering Party, 7 days a week from 7:00 to 21:00 .In this estimated number of hours in 2022 year - 58 100 hours, while in 2023 year - 41,500 hours. Such hours shall be accepted for the calculation of the offer price. Whereas under the terms of the dimension of the hours of provision of the Services, only the actual time of provision of the Services should be understood. The contracting person understands the clock hour i.e. 60 minutes.

1. The subject of the contract is the provision of specialized care services for adults with mental disorders from Gdynia, in the following areas of activity:

(a) DOPS district no. 1 — Districts: Redłowo, Orłowo, Karvinas, Dąbrowa, Mały Kack, Wielki Kack, part of St. Maximilian Hill, part of Wiczlin.

(b) DOPS District No 2 — Districts: Oksywie, Obland, Foothills;

(c) DOPS district no. 3 — Districts: Grabówek, Leszczynki, Chylonia, Pustki Cisowskie, Demptowo, Cisowa;

d) DOPS District No. 4 — Districts: Downtown, Kamienna Góra, part of St. Maximilian Hill, Forest Plots, Witomino, Chwarzno — Wiczlino.

2) The Contracting Authority reserves the opportunity to reduce the expected max. dimension of the hours provided by the Services, in particular depending on the number of persons applying for the aid given form or incomplete security of funds for the implementation of the Services by the Pomeranian Voivodeship. The above number of hours of the Services is max., while the minimum time dimension of Services provided during the period of completion of the subject of the order is 65,000 hours. The contractor is not entitled to claim the order max.number of hours. In the event of an order of an incomplete number of hours, the Contractor will only be entitled to the remuneration resulting from the completed Services.

3) Detailed information on the subject matter of the contract and the method of its implementation shall be determined by itch no. 1 to SWZ - Gdynia standard of specialized care services provided at the place of residence, draft contract which is annex 2 to SWZ.

2. The Ordering Party does not reserve the obligation to personally execute the key parts of the order by the Contractor. The Contractor may entrust the execution of part of the subcontractor's order In such a situation, the Contracting Authority shall require the Contractor to indicate the parts of the contract, the execution of which it intends to entrust to the Subcontractors and provide by the Contractor the names of the Subcontractors (where known).

The Contracting Authority shall not reserve the possibility of applying for a contract only by the Contractors referred to in Article 94 of the Pzp Act.

The Contracting Authority does not provide for the Contractor's obligation to perform a local vision and check by the Contractor of the documents necessary for the execution of the order available on site with the Contracting

The Contracting Authority shall not permit the possibility of submitting a variant offer referred to in Article 92 of the Pzp Act, i.e. an offer providing for a different manner of execution of the contract than specified

in this SWZ.

The Contracting Authority does not provide for the conclusion of a framework agreement and the establishment of a dynamic purchasing system and does not provide for the selection of the most advantageous offer using an electronic auction.

The contracting authority does not provide for the award of contracts pursuant to Article 214 (1) point 8 of the Pzp Act.

The Contracting Authority shall not allow tenders in the form of electronic catalogs or the attachment of electronic catalogs to the tender, in the situations referred to in Article 93 of the Pzp Act.

The Ordering Party does not provide for settlement in foreign currencies. The settlement of the order is made in PLN.

The awarding entity does not provide for reimbursement of the costs of participation in the procedure.

The awarding entity does not provide for advance payments for the performance of the contract.

Due to the nature of the contract, the Contracting Authority does not require employment by the Contractor or Subcontractor, on the basis of the employment relationship, of persons performing the activities

during the execution of the order

The Ordering Party shall not allow the possibility of submitting partial tenders (the order is not divided into parts). This order is an inextricable whole for organisational, economic reasons. The Contracting Authority performs orders for various guardianship services, dividing them in terms of their specialized nature and taking into account the welfare of the wards.

The contracting authority does not require the lodging of defects in the present proceed.The TOTAL OFFER PRICE GROSS PLN The Contractor calculates as follows: Estimated Number of hours in 2022 i.e. 58100 hours times offered unit price per 1 hour in 2022 plus Estimated Number of hours in 2023
