The subject of the contract is the supply of inhalation anesthetics along with the lending of evaporators for SPZOZ WSS No. 3 in Rybnik. In addition, the contractor is required to lend (for the entire period of the contract) the equipment necessary to administer the drug – evaporators, specified in detail in Appendix 1.1.A on the terms set out in the contract. The order is divided into 2 packages (parts). The delivery specification is included in Appendix 1.1, i.e. "Price Form" and Appendix 1.1.A, i.e.: "List of anaesthesia machines owned by the Contracting Authority and evaporators lent by the Contractor" to this Specification of Contract Conditions. Other terms and conditions of the contract are set out in Appendix 2 to the terms of reference ("Draft provisions of the contract", on public procurement to be incorporated into the contract). The agreements for the individual packages will be concluded after the end of this procedure for a period of 36 months.
