1. The subject of the contract is the provision of early development support services for children as part of a comprehensive support program for families "For Life". Early development support services will be provided in the form of individual classes with children from birth to the time of starting school. One hour of early development support is a full clock hour (60 minutes). The estimated number of hours to be completed as part of pedagogical therapy classes is in the range of 150 - 200 hours.
1. The subject of the contract is the provision of early development support services for children as part of the comprehensive support program for families "For Life". Early development support services will be provided in the form of individual classes with children from birth to school. One hour of early development support is assumed to be a full clock hour (60 min.).
2. The estimated number of hours to be completed as part of pedagogical therapy classes is in the range of 150 - 200 hours.
3. The scope of duties of a specialist conducting individual pedagogical therapy for children includes:
- after being appointed to a team of specialists for a specific child, determining the directions and schedule of activities undertaken in the field of early support and support for the family,
- establishing cooperation with the institution where the child is covered by other therapeutic activities in order to ensure consistency of interactions,
- developing and implementing with the child and his family an individual program of early development support in the part intended for the educator,
- organizing meetings and consultations for the child's parents, legal guardians of the child, actual guardians of the child,
- assessing the progress and difficulties in the functioning of the child, including identifying and eliminating barriers and restrictions in the environment hindering his activity and participation in social life,
- analysing the effectiveness of the assistance provided under the programme to the child and his or her family,
- making changes to the program in accordance with the current needs of the child and his family,
- planning further early support activities.
4. Obligation to employ under an employment contract. The Contracting Authority requires that the Contractor or Subcontractor employs persons performing the activities indicated in point 3 on the basis of an employment contract throughout the entire period of performance of the subject of the contract within the meaning of the provisions of the Act of 26 June 1974 – Labour Code. The number of full-time jobs must correspond to the time necessary for employees to perform all activities covered by the early development support service. The above condition will be met by employing new employees under a contract of employment or appointing employees already employed by the Contractor to perform the contract. NOTE: the requirement does not apply to contractors who are natural persons who will perform the subject of the contract personally.
