1. The subject of this order are successive supplies of suture materials, resin material, bone wax for the Oncology Center of the Institute. Maria Skłodowska-Curie Branch in Gliwice.
2. The subject of the contract is divided into 5 separate tasks.
3. Description of the subject of the contract and the conditions of the contract are included in the forms:
a) Assortment and price specification - Annexes 2.1– 2.5 to the ToR;
b) Contract template - Annex 4 to the ToR.
4. In order to assess the quality, the Purchaser requests submission of free samples in the assortment
And the quantities indicated in Annex 5 to the ToR.
5. The Awarding Entity does not envisage concluding a framework agreement, establishing a dynamic purchasing system, electronic auction and supplementary orders.

5. Samples are submitted for quality assessment and in this case are not considered as a document
Within the meaning of art. 25 paragraph 1 of the Public Procurement Law, and thus art. 26 section 3 of the Public Procurement Law will not apply. Samples will not be replenished. Lack of submission of samples, submission of samples after the deadline specified in chapter XIV of the ToR, submission of samples in a different place or in a different form than indicated in the ToR, or submission of samples that do not comply with the requirements of the Ordering Party or do not comply with the product offered in the product and price specification will result in rejection of the offer .
6. Contractors whose offer has not been selected in the public procurement procedure have the right to return samples upon a written request. The Purchaser reserves that they are not returnable samples that will be partially or completely used in the quality assessment process, and / or the relevant regulations and procedures prescribe their safe disposal.
7. The Purchaser does not allow the submission of variants.
