The subject of the contract is the delivery of knee joint endoprostheses along with the rental of a system for knee arthroplasty and the supply of consumable materials, including:

A. Delivery of knee replacements:

1. The subject of the contract is the delivery of knee joint endoprostheses to the Contracting Authority's headquarters (Cracow, 1 Złota Jesień Estate), in accordance with the description and requirements contained in Appendix No. 1A to the Contracting Authority. SWZ.

2. Offers that do not have the full scope of the item of the order will be rejected.

3. The remaining terms of the contract are defined by the draft contract, as set out in Annex 3A to the SWZ.

B. Renting a knee replacement system:

1. The subject of the contract is the lease of a system for knee arthroplasty, in accordance with the description and requirements contained in Appendix No. 1B to the NIN. SWZ.

2. Offers that do not have the full scope of the item of the order will be rejected.

3. The remaining terms of the contract are defined by the draft contract, which is annex No 3B to the SWZ.

C. Supply of consumables necessary to perform arthroplasty procedures:

1. The subject of the contract is the supply of consumables necessary to perform arthroplasty procedures, in accordance with the description and requirements contained in Appendix No. 1C to the NIN. SWZ.

2. Offers that do not have the full scope of the item of the order will be rejected.

3. The remaining terms of the contract are defined by the draft contract, which is Annex 3C to the SWZ.
