1. The subject of the contract includes the sale and delivery of medical equipment for the needs of hospital wards of Szpital w Ostróda S. A., hereinafter also referred to as devices, as part of the following PARTS of the contract:

1) PART 1 – sterilizer – 1 pc,

2) PART 2 – laser for urological procedures - 1 pc,

3) PART 3 – operating table – 1 pc,

4) PART 4 – anaesthesia machine – 1 pc,

5) PART 5 – diagnostic and treatment chair – 1 pc,

6) PART 6 – gynaecological chair – 1 pc.

2. A detailed description of the subject of the contract covered by PART 1 - 6 of the contract is included in Annex 3-8 to the tender specifications, hereinafter referred to as SWZ.

PART 1: Sterilizer - 1 pc- - a detailed description of the subject of the contract covered by PART 1 of the order can be found in Annex 3 to the SWZ.

PART 2: Laser for urological procedures - 1 pc - a detailed description of the subject of the contract covered by PART 2 of the order can be found in Annex 4 to the SWZ.

PART 3: Operating table - 1 pc - a detailed description of the subject of the contract covered by PART 3 of the order can be found in Annex 5 to the SWZ.

PART 4: Anaesthesia apparatus - 1 pc- - a detailed description of the subject of the contract covered by PART 4 of the order can be found in Annex 6 to the SWZ.

PART 5: Diagnostic and treatment chair - 1 pc - a detailed description of the subject of the contract covered by PART 5 of the order can be found in Annex 7 to the SWZ.

PART 6: Gynecological chair - 1 pc - a detailed description of the subject of the contract covered by PART 6 of the order can be found in Annex 8 to the SWZ.
