1. The subject of the contract includes the successive delivery of orthopaedic endoprostheses and implants under the following parts: from part 1 to part 15.

2. A detailed description of the individual parts of the contract can be found in the assortment and price form – Appendix No. 2 to the Terms of Reference.

3. During the period of performance of the contract, the Contractor shall provide the Contracting Authority with free of charge complete instruments for implanting and removing the assortment which is the subject of the contract and shall provide its service.

4. For the duration of the order, the Contractor is obliged to provide the Contracting Authority with a deposit (for the Operating Theatre) of at least 1 complete line of implants covered by the order to be used in accordance with the current demand.

5. The Contractor shall deliver the components of the deposit and the lent instruments together with the handover protocol within 7 working days from the date of signing the contract. The Contractor shall attach to the delivery an up-to-date model of the consumption protocol for individual parts of the contract.

Part 1-endoprosthesis total of the knee joint, cement free primary and revision: size and scope are specified in the assortment and price form set out in Annex 2 to SWZ.

PART 10 – Implants for satbilization of the cervical spine with anterior approach – size and scope are specified in the assortment and price form constituting Appendix No. 2 to the Terms of Reference.

PART 11 – meniscus sewing kit – size and scope are specified in the assortment and price form constituting Appendix No. 2 to the Terms of Reference.

PART 12 – implants for arthroscopic procedures – size and scope are specified in the assortment and price form constituting Appendix No. 2 to the Terms of Reference.

PART 13 – bone cement – size and scope are specified in the assortment and price form constituting Appendix No. 2 to the Terms of Reference.

PART 14 – cementless hip endoprosthesis – size and scope are specified in the assortment and price form constituting Appendix No. 2 to the Terms of Reference.

PART 15 – resin elements for the arthroscopic tower – size and scope have been specified in the assortment and price form constituting Appendix No. 2 to the Terms of Reference.

PART 2 – Primary knee joint endoprosthesis: the size and scope are specified in the assortment and price form constituting Appendix No. 2 to the Terms of Reference.
