1. The subject of the contract is the adaptation of the Ordering Party's domain systems to the processing of EDM in accordance with the requirements of the Act on the information system in health care and the exchange of data with e-services for patients offered as part of the e-services Platform implemented as part of the project "e-Health in SP ZOZ MSWiA: development of modern public e-services for patients" under the second axis of the OP PC, Measure 2.1 "High availability and quality of public e-services.".

1. The subject of the contract is the adaptation of the Ordering Party's domain systems to the processing of EDM in accordance with the requirements of the Act on the information system in health care and the exchange of data with e-services for patients offered as part of the e-services Platform implemented as part of the project "e-Health in SP ZOZ MSWiA: development of modern public e-services for patients" under the second axis of the OP PC, Measure 2.1 "High availability and quality of public e-services.". The adaptation of the systems will include:

1) Expansion / delivery of Domain Systems in the scope of the following modules:

2) Adaptation and integration of the Ordering Party's domain systems with the Medical Information System in accordance with the requirements specified by the e-Health Center in the field of exchange of electronic documents with central systems in the scope of:

a) Electronic Medical Records

b) Medical events

c) e-referrals

d) e-recipes

3) Adaptation and integration of the Ordering Party's domain systems in the field of data exchange with the e-services Platform implemented as part of the e-Health project in the SP ZOZ MSWiA: development of modern public e-services for patients" in accordance with the requirements set out in the SWZ in the field of services:

a) e-EDM

b) e-Registration

c) e-Analysis

4) Delivery of the Local Repository of Electronic Medical Records system together with indefinite licenses payable once for the use of the system that meets the functional and non-functional requirements specified in the SWZ.

5) Delivery of system software necessary for the operation of the supplied systems.

6) Delivery of the database engine based on which the HIS system is to operate together with the necessary number of licenses to work the above-mentioned software on servers provided as part of the order and migration of data from the Database of the System currently used by the Ordering Party.

7) Delivery of certificates necessary for the operation of the system, in particular for the exchange of data within the SIM and for connection to the National Node.

8) Installation, implementation, configuration and launch of the above-mentioned software on IT equipment indicated by the Ordering Party.

9) Providing test documentation (Test Plan, Scenarios and Test Cases) for the software being delivered

10) Conducting acceptance, integration, performance and safety tests

11) Training of the Ordering Party's staff in the use of the above-mentioned application software and database software, server operating systems

12) Providing the documentation required in the SWZ

13) Warranty for the delivered software
