Barnsley Clinical Commissioning Group would like to commission a new model for Social Prescribing services. The new model will enhance the health and well-being of the residents of Barnsley and help decrease health inequalities, by providing a liaison service to support patients and social care client's to access non-medical sources of support in the community. The Borough wide social prescribing service for the population of Barnsley, will be branded ‘My Best Life’, which is reflective of the ethos behind it and is well-understood and promoted.
