1. The subject of the contract is the lease of a surgical robot along with the supply of consumables adapted to perform radical prostatectomy procedures using a robotic system for the needs of the John Paul II Provincial Hospital in Bełchatów.

3. Description, size and scope of the item of the order:
1) The subject of the contract is the lease of a surgical robot together with the supply of consumables adapted to perform radical prostatectomy procedures using a robotic system.
2) The device being the subject of the lease meets the requirements set out in the law, in particular in the Act on medical devices and in accordance with the Regulation of the Minister of Health of 25 January 2022 amending the Regulation on guaranteed services in the field of hospital treatment (Journal of Laws of 2022, item 245) may be used in the surgical treatment of prostate cancer using a robotic system.
3) The subject of the contract must ensure the performance of 100 robotic treatments during the term of the Agreement, while the Ordering Party reserves the right to complete the full scope of the contract within less than 12 months. As part of the implementation of the subject of the contract, the Contractor is obliged to:
a. lease to the Ordering Party an efficient and complete surgical system by making the device available for treatment sessions;
b. sale and supply of consumables – necessary tool kits, disposable robotic sleeves and accessories to perform treatments;
c. training of the Ordering Party's operational team.
4) A detailed description, including the parameters required and evaluated, are set out in Annex 5 to the SWZ.
5) The subject of the contract offered in the procedure has all the necessary certificates, including declarations of conformity, complies with the standards in force in the European Union, meets the requirements set out in the Act on medical devices and the marketing and use approvals required by overarching regulations.
6) Other requirements for the performance of the contract and the rules of cooperation between the contracting authority and the contractor are specified in the draft provisions of the public procurement contract, constituting Annex 3 to the SWZ.
