1 The subject of the contract is group life insurance for employees and family members of employees of the Independent Public Health Care Institution in Bochnia "District Hospital" named after Blessed Marta Wiecka.

2. The Contracting Authority does not guarantee that all employees employed and members of the families of employees of the Ordering Party will enter into group life insurance.

3. During the period of insurance, there may be changes in the number of insured persons.

4. If an employee or a family member of an employee resigns from insurance, the Employer is not obliged to pay an insurance premium for them.
