1. The subject of the order is the delivery of non-sterile diagnostic gloves.

2. The subject of the contract has been divided into lots. The Contracting Authority shall allow the submission of partial tenders.

3. A detailed description and method of execution of the order shall be provided by OPZ, constituting Annex No 1-3 to the SWZ.

4. The Contracting Authority shall not allow equivalent tenders.

5. The Contracting Authority shall require that: the expiry date for the delivered object of the order shall be at least 12 months from the date of delivery of the goods to the office of the Contracting Authority.

6. The particulars strictly required for the proper identification of the device shall be given in each

(or on each) of the packaging and on the outer packaging, and must contain at least:

(a) the trade name,

b) marking the size of the gloves,

(c) the batch number,

(d) the expiry date/date.

e) each individual package must contain instructions for use in Polish.
