The London Borough of Ealing is seeking to implement an Integrated Lifestyle Service which will be procured in 3 Lots.

Lot 1: Stop Smoking Service, Nicotine Replacement Therapy (NRT) and Community NHS Health Check Outreach Service;

Lot 2: Child Weight Management and Health Walks Service;

Lot 3: Health Trainers Programme, TB Awareness Raising Programme, ESOL — Skills for Health and the Hub. The Hub will establish a single point of entry to the integrated lifestyle services and will offer a central booking system. The hub will be responsible for co-ordinating and receiving referrals, providing triage for the integrated lifestyle service which includes the Smoking cessation service, NHS health checks, Health walks, Child weight management programme and referrals to a wide range of other support services.

A separate contract will be awarded for each of the 3 Lots of services.
