The subject of the contract is the supply of reagents, CALIBRATORS, CONTROL AND CONSUMABLE MATERIALS NECESSARY FOR THE OPERATION OF LEASED AUTOMATIC ANALYZERS

FOR IMMUNOCHEMISTRY TOGETHER WITH THE LEASE OF AUTOMATIC ANALYZERS FOR IMMUNOCHEMISTRY in accordance with the description and requirements contained in Appendices No. 2, i.e. in the assortment – price, price form and in Appendices No. 10 under the name "Summary of required technical parameters" to the Specification.

Partial offers:

1.The subject of the contract has not been divided into parts and therefore the offer should be submitted to the whole.

2.In the assortment and price form (Appendix No. 2.1.) and price form (Appendix No. 2.2), regarding the lease of automatic analyzers for immunochemistry) all items must be included. The lack of any assortment item of Appendix No. 2.1. and 2.2. will result in the rejection of a given tender offer.

Required period for execution of the object of the contract (contract) and conditions for execution of the

1. Deliveries of the subject of the order will take place successively, according to the needs of the Ordering Party in accordance with the assortment and price form (Appendix No. 2.1.) in the form of partial deliveries.

2.The period of execution of the object of the contract (contract) shall be for a minimum period of 24 months from the date of conclusion of the contract or until the amount constituting contractual remuneration is exhausted.

3. Deliveries of the subject of the order will take place on working days from 8.00 to 14.00 according to written orders placed by e-mail by the Head of the Laboratory Diagnostics Department or another authorized person.

In very urgent and justified cases "on cito" orders can be placed by phone.

4. In connection with various situations, e.g. emergencies, epidemics, etc., affecting the implementation of the subject of the contract of future contracts, as well as in the event of non-exhaustion of the assortment, the Ordering Party allows the extension of the time of implementation of the subject of the contract for a period not longer than 12 months or until the assortment is exhausted.

5. The minimum delivery time of the subject of the order is two working days, while the maximum

the delivery time is seven working days from the date of placing the order (the criterion for the evaluation of tenders, which is described in Chapter XXI).

6.Automatic analyzers for immunochemistry described in Appendices No. 10 The Contractor is obliged to deliver at his own expense and risk to the "Medical Center in Łańcut" Sp. z o. o. to the Laboratory and put them into operation within a maximum period of up to eight weeks from the date of signing the contracts (on working days).

7. The delivery period is counted on the next business day from the date of placing the order by

Ordering man.

8. The Ordering Party agrees that working days are days from Monday to Friday from 8.00 to 14.00, excluding Saturdays and Sundays and holidays specified by law.

Term, form of payment:

1. Payment deadline – payment for partial implementation, i.e. for the delivery of the subject of the contract, i.e. for the delivery of reagents, calibrators, control and consumable materials necessary for leased analyzers for immunochemistry will be made by bank transfer from 30 to 60 days (minimum payment term 30 days, while the maximum payment term 60 days) after delivery and receipt of a properly issued VAT invoice (payment deadline that will be valid for the period of validity The Contractor is obliged to enter the contract in the offer form, i.e. in Appendix No. 1).

2.Payment deadline for immunochemistry analyzers – payment for the lease of each automatic immunochemistry analyzer will be made in installments for a period of 24 months within 30 days starting from the first month in arrears, after the month in which the Contractor will put the automatic immunochemistry analyzers into operation and submit

at the Ordering Party a correctly issued VAT invoice.

3.Form of payment – transfer.

4.Where payment is due on a public holiday or Saturday, payment shall be made on the first working day following those days.

5. Invoices in electronic form are obliged to be sent by the Contractor only to the following address: faktury.lab@cm-lancut.pl.
