Description:
Purchase of magnetic resonance imaging apparatus, PACS system with its installation and integration with the HIS hospital system and reconstruction and expansion of the magnetic resonance imaging and computed tomography laboratory

1. The object of the order is:

1) delivery, installation, assembly, commissioning of the magnetic resonance imaging device along with the installation of the PACS system and integration with the hospital HIS and RIS system, staff training; a detailed description of the technical parameters of magnetic resonance imaging is specified in Annex 2 a, which is an integral part of the SWZ

2) reconstruction with extension of the rooms for the purpose of creating the Magnetic Resonance Imaging Laboratory (RM) and computed tomography (CT) of the Independent Public Health Care Institution in Hajnówka. All works in the scope fall within the material scope of the Contractor. The minimum scope of work that the Contractor should perform is included (Annex 2 c – PFU.):

2. The order will be completed in a comprehensive and complete manner, enabling the use of the premises of the RM and TK laboratories and RM devices together with the necessary turnkey system and equipment, without the need for the Ordering Party to incur additional costs.

1. Before commencing the construction works of the RM and TK rooms, the Contractor will be obliged to prepare design and executive documentation of the Magnetic Resonance Imaging and Komputra Tomography Laboratory, taking into account the organization of the laboratory resulting from the requirements of the offered RM system and relevant regulations regarding the RM and TK laboratories. The design and executive documentation must be agreed with the Ordering Party and experts for sanitary and hygienic matters, occupational health and safety and fire protection and ensure that the Ordering Party obtains a permit to use the RM workshop from the State District Sanitary Inspector.

2. After completion of the works – the Contractor will carry out measurements of the electromagnetic field and provide documentation containing plans of rooms along with marked zones and measurement results. The Contractor is obliged to properly shield the rooms for the offered device (faraday cage) in compliance with European and Polish utility standards and health and safety rules. The Contractor shall make every effort to obtain by the Ordering Party from sanepid a permit to use the entire Magnetic Resonance Imaging Laboratory.

3. On the side of the Contractor/Supplier, it is also necessary to prepare the magnetic field line marking in the RM laboratory and adjacent rooms, in accordance with the manufacturer's recommendations and applicable law on the territory of the Republic of Poland and the EU.

4. The Contractor will replace the PACS, the requirements for replacement are described in Annex 2 b to the SWZ.

5. The Contractor shall integrate PACS with the His/RIS OPTIMED system of Comarch owned by the Ordering Party.

6. Delivery of the subject of the contract together with transport, unloading, full installation of magnetic resonance imaging and equipment necessary for the functioning, foundation, integration and construction works will take place entirely at the expense and risk of the Contractor.

7. The offered subject of the order should be brand new, free from physical and legal defects, and after delivery to the Ordering Party, ready for use in accordance with its intended purpose, without the need for the Ordering Party to incur additional financial, organizational and technical outlays beyond those resulting from their normal operation, and not constituting the subject of this order.

8. The place of delivery of the equipment is the seat of the Ordering Party - SP ZOZ in Hajnówka, at 9 Doc. Adama Dowgirda Street, rooms indicated by the Ordering Party, after prior telephone arrangement.

9. The subject of the contract includes training for the Ordering Party's staff with obtaining a certificate. Detailed rules for conducting the training are described in the essential terms of the contract (Annex 4 to the SWZ)

10. The subject of the order must be complete and after installation / assembly and commissioning ready to work without the need for the Ordering Party to incur additional financial, organizational and technical outlays beyond those resulting from their normal operation and not being the subject of this order.
