1. The subject of the contract is group life insurance for employees and members of the families of employees of the John God Self-Public Provincial Hospital in Lublin.

2. The Contracting Authority does not guarantee that all employees employed and members of the families of employees of the Ordering Party will enter into group life insurance.

3. There may be changes in the number of insured persons during the contract period.

4. If an employee or a family member of an employee resigns from insurance, the Employer is not obliged to pay an insurance premium for them.
