3.1. The subject of the contract is the delivery and assembly along with the design and commissioning of 37 solar collector installations under the program "The use of solar energy in the Municipality of Nowe Ostrowy" on private real estate and public buildings located in 21 towns of the Municipality of Nowe Ostrowy.
3.1.1. As part of the contract, the contractor will prepare design documentation for 37 solar collector installations on private buildings or on land belonging to the commune residents, together with the delivery of installation components and on-site assembly of solar collector installations based on the completed project documentation, including commissioning and user instructions. Two types of solar collector installations are envisaged (including one subtype), depending on their capacity and volume of storage heaters (sets), in quantities:
• Set I: 2 flat solar collectors with a capacity of min. 2690 W, feeding the DHW cylinder with a net volume of min. 240 dm3 in quantity of 21 items
• Set Ia: 2 flat solar collectors with a capacity of min. 2690 W, feeding the DHW cylinder with a net volume of min. 280 dm3 in quantity of 1 item
• Set II: 3 flat solar collectors with a capacity of min. 4035 W, feeding the DHW cylinder with a net volume of min. 280 dm3 in the amount of 15 items

8. List of statements or documents confirming the fulfillment of the conditions for participation in the procedure and no grounds for exclusion
To confirm the conditions of participation in the procedure and the lack of grounds for exclusion, the contractors shall submit:
8.1. Along with the offer, JEDZ's statement in electronic form with a qualified electronic signature in order to pre-confirm the conditions of participation in the procedure and the lack of grounds for exclusion. Statements of entities submitting the offer jointly and entities sharing the potential submitted on the JEDZ form should be in the form of an electronic document, signed by a qualified electronic signature by each of them to the extent that they confirm the circumstances referred to in art. 22 (1) of the PZP Act, i.e. meeting the conditions for participation in the procedure and no grounds for exclusion to the extent that each of the contractors shows that they meet the conditions for participation in the procedure and no grounds for exclusion (Contractors must jointly demonstrate compliance with the conditions for participation in the procedure, but no grounds each is excluded separately).
8.1.1. The Purchaser allows in particular the following format of sent data: .pdf, .doc, .docx, .xps, .odt, rtf.
8.1.2. The contractor fills in EAT, creating an electronic document. Can use the ESPD tool or other available tools or software that allow you to fill in the EAT and create an electronic document, in particular in one of the abovementioned formats.
8.1.3. After the contractor has created or generated the EAT electronic document, the contractor signs the abovementioned a document with a qualified electronic signature issued by a qualified trust service provider that is an entity providing certification services - an electronic signature that meets the security requirements set out in the Act.
8.1.4. The contractor is obliged to complete the EAT form in the scope of: Part II, sections: A, B, C, D; Part III sections: A, B, C, D; Part IV section: α; Part VI - signature.

8.2. Within 3 days of posting on the website the information referred to in art. 86 section 5 of the Public Procurement Law, the Contractor shall provide the Employer, without additional request, with a statement
On belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Public Procurement Law in order to demonstrate the lack of grounds for exclusion from the public procurement procedure in the circumstances referred to in art. 24 paragraph 1 point 23) of the PZP Act, according to template No. 4 to IDW ....
