Supply of antibiotics and other anti-infectious drugs for a hospital pharmacy warehouse.
PN 08/24-SUPPLY OF ANTIBIOTICS AND OTHER ANTI-INFECTIOUS DRUGS

1. The subject of the contract is the supply of antibiotics and other anti-infectious drugs - according to the division into 25 TASKS

2. Detailed description of the subject of the order, including the required form, dose, quantity op., quantity op. specifies the ASSORTMENT AND PRICE FORM, constituting ANNEX No. 1 to the SWZ.

3. Series and expiry date of products on the packaging and on the invoice, expiry date of not less than 12 months.

4. Delivery date - max 2 working days from the date of placing the order.

5. The offered medicinal product must be authorized for marketing and use in the Polish for each required form and dose of the drug separately - in accordance with the Act of 06.09.2001. Pharmaceutical Law.

6. The detailed conditions and rules for the implementation of the subject of the contract are further defined by the contractual conditions of Annex 5 to the SWZ “Contractual Terms”.

7. The Contractor shall deliver the objects of the order to the Pharmacy Warehouse of the HOSPITAL.
