Description:
Reception and management of municipal waste from Selective Municipal Waste Collection Point in Krzeszowice, at 7 PoTransczna Street (PSZOK)

1. The subject of the contract includes the collection and management of municipal waste from the Selective Collection Point of Municipal Waste in Krzeszowice, at 7 PoTransczna Street (PSZOK).

Types of waste covered by the subject of the order and their estimated quantities (in Mg):

Biowaste - 200;

Plastics, plastic packaging - 50;

Furniture and other bulky waste - 420;

Glass - 35;

Insulating materials - 30;

Total: 735.

The Contracting Authority stipulates that the above values may be used to determine the value of the offer, however, the quantity of waste may change depending on the actual amount of waste generated by the owners of the property.

3. The detailed scope of the service and any requirements for the provision of the service for the collection and management of municipal waste covered by this order (requirements set out pursuant to Article 218 (2) of the Act) are described:

- Part II — Detailed Description of the Subject of the Order and

- Part III — Model Agreement.

1. The subject of the contract includes the collection and management of municipal waste from the Selective Collection Point of Municipal Waste in Krzeszowice, at 7 PoTransczna Street (PSZOK).

2. The item of the order also includes:

a. delivery to the PSZOK area of an uncovered container, type KP 30 (with a capacity of minimum 30 m3), adapted for transport by car “hook”. Container will be designed for collection

in it biowaste (green waste). During the season of occurrence of an increased amount of biowaste (from spring to autumn), you may need to substitute two such containers on PSZOK.

b. protection of waste during transport. When providing a waste transport service from PSZOK to the installation, the Contracting Authority requires that the Contractor ensure proper protection of transported waste (including tarpaulins, containers mesh) against the impact of atmospheric factors and protection in a way that prevents the spread of waste beyond means of transport, in particular its dumping, dusting, leakage, in accordance with the Regulation of the Minister of the Environment of 7 October 2016 on detailed requirements for the transport of waste.

c. The Contractor is obliged to supply bags for selective collection of municipal waste (yellow — for plastics, multi-material packaging and metal; green — for glass; blue — for paper and cardboard) to the Municipal Waste Selective Collection Point, where they are handed, for replacement, to residents supplying waste to segregated fractions.

In addition, the service includes substitution of containers for PSZOK for exchange. During the provision of the waste collection service from PSZOK, if necessary, the Ordering Party requires the substitution of the container by the Contractor, for replacement, with parameters similar to the Ordering Party's container handed over for emptying. Such a need may occur in the case of a longer wait for the return of the container, collected for emptying of waste. The demand for the container will be forwarded at the time of reporting the need to collect waste – electronically. The provision of containers for exchange may concern a maximum of 4 pcs., with parameters similar to the containers at the disposal of the Ordering Party, specified in Chapter B, point 2 of the Detailed Description of the Subject of the Order Part II to the SWZ.

2. The Contracting Authority requires the submission with the offer of the following evidence measures in question:

(a) Photographs of cars (whole vehicle) listed in Chapter 9, paragraph 1.4, point 2), (a) together with their description, resulting from which all the conditions listed in Chapter 9, paragraph 1.4, point 2), (a) will be fulfilled.

b) Document*, which will clearly show that the vehicle used by him adapted to receive mixed municipal waste has a specific EURO 5 or EURO 6 emission standard or equivalent or is a vehicle powered by alternative fuels within the meaning of the Act on electromobility and alternative fuels.

In particular, the Contractor may submit one of the following documents:

1) EURO certificate,

2) vehicle registration certificate,

3) vehicle card,

4. an extract from the approval certificate or a copy of the decision exempting the vehicle from approval,

5) a statement of the manufacturer or an authorized representative of the manufacturer,

6) CE conformity document - a document issued by the vehicle manufacturer or his authorized representative, constituting a legally binding promise confirming the compliance of the vehicle with the essential requirements of the relevant European Union Directives together with a copy of the registration certificate with the same VIN number (possibility of unambiguous identification of the vehicle)

3. In connection with paragraph 2, the Contracting Authority provides for the application of Article 107 (2) of the Pzp
